PRN Nurse Practitioner

This position will serve Humboldt Park, River Grove, Irving Park and the surrounding areas

Schedule: 1 day per week 

$125 per completed visit

Key Responsibilities:

• Conduct in-home visits, health assessments, and care planning for members with acute and chronic conditions


• Collaborate with other healthcare providers to deliver high-quality, patient-centered care


• Participate in call rotation of telephonic on call coverage after hours

Requirements:

• Bachelor’s in nursing with Illinois APRN license (Master’s preferred).


• Board certification for Nurse Practitioners and DEA licensure.


• Valid driver’s license and proof of insurance.
