Arizona Home Care, the largest locally owned Medicare Certified Home Health Agency in Phoenix, is looking for a skilled Occupational Therapist to join our team on a PRN basis in Scottsdale, AZ. Position Highlights:

• Pay: $90 per unit
• Work Type: PRN (as-needed basis)
• Location: Scottsdale, AZ
• 1-2 years of Home Health experience required
• Fluency in OASIS is required (KanTime EMR knowledge is helpful but not required)

Key Responsibilities:

• Conduct comprehensive assessments of patients’ needs in their home environment
• Develop and implement individualized treatment plans to enhance patients' ability to perform daily activities
• Provide skilled occupational therapy services, adjusting interventions as necessary to achieve patient goals
• Educate patients and caregivers on strategies to improve functional mobility and independence
• Ensure timely and accurate documentation in compliance with OASIS and regulatory standards
• Collaborate with a multidisciplinary team to deliver optimal care
• Maintain positive, professional relationships with patients, families, and colleagues

Requirements

Qualifications:

• Active Occupational Therapist license in Arizona
• 1-2 years of Home Health experience required
• Fluency in OASIS required (KanTime knowledge a plus)
• Arizona Level One Fingerprint Clearance Card required
• Valid Driver's License and car insurance card
• Current CPR and First Aid certification required
• Current TB test
• Two professional references required at time of interview
