PRN PT Responsibilities:

• Patient Initial Examination/Assessment, Progress notes, Discharges, Free screens
• Provide patient centered treatment per IN State Practice Act
• Document and Bill for services provided per payer rules
• Patient outcomes and completion of POC
• Communicate with referring healthcare providers, payers, patients and their family members.
• Direct and delegate to PTA/aide as appropriate 
• Maintain continuing competencies and training staff
• Collaborate with the team to provide Amazing patient experience and follow patient communication sequences.
• Educate other healthcare providers about physical therapy
• Maintain and comply with patient confidentiality and integrity in all patient matters
• Promote events and services offered
• Promoting healthy lifestyle, wellness and injury prevention strategies in the community
• Other duties as directed by the clinic owner, and administrator

At Chesterton Physical Therapy, we believe in giving back to the community so willingness to participate in charitable activities and donate time is expected. Occasional coverage for Saturday will be required.

Requirements

• Physical Therapy degree from an accredited institution
• Current state licensure in physical therapy
• Knowledge and experience in physical therapy clinical assessments and treatment
• Excellent written and verbal communication skills
• High level of professionalism and strong ethics
• Experience with electronic medical records (EMR)
•  Ability to demonstrate clear, rational and defensible clinical reasoning 
• Knowledge of healthcare standards of practice and regulatory health requirements
