Responsibilities
- Conduct assessments to determine the social, emotional, and environmental needs of clients.
- Develop and implement care plans tailored to the individual needs of clients.
- Provide counseling and support to clients and their families.
- Collaborate with healthcare professionals to ensure comprehensive care.
- Advocate for clients and connect them with community resources and services.
- Maintain accurate and timely documentation of services provided.
Requirements
- Master's degree in Social Work from an accredited institution.
- Valid and current social work license in the state of Florida.
- Previous experience in a healthcare or home care setting is preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Flexibility to work on an as-needed basis, adapting to the varying needs of clients.
- Valid driver’s license with reliable vehicle and auto insurance
