Job Responsibilities:
Explore dreams, desires, and goals with people.
Support people to achieve their goals, dreams, and desires.
Develop the Person-Centered Support Plans (PCSP) for people as part of the interdisciplinary team:
Complete the self-assessment, risk assessment, and other needed assessments before the PCSP meeting.
Schedule and chair the PCSP meeting.
Develop objectives, support statements, goal statements, and other action items needed.
Write the final plan, including meeting minutes, and distribute this information within ten working days of the PCSP meeting date.
Implement and coordinate the Person-Centered Support Plan. This would include the use of Therap.
Determine the effectiveness of the Person-Centered Support Plan and make accommodations as needed.
Complete and maintain all necessary paperwork for the Human Rights Committee and Behavior Management Committee, including monthly documentation as requested by the committees.
Initiate and maintain eligibility for financial benefits for people.
Provide crisis intervention and counseling to people and assess challenging situations. This includes making external referrals for additional support as needed.
Provide ongoing support and problem-solving daily.
Establish a positive, trusting relationship with each person supported; encourage communication with each person.
Initiate and maintain ongoing communication with parents/guardians and other family members.
Participate in the on-call rotation and perform all duties as necessary.
Adhere to Red River Human Services Foundation policies and procedures, personnel policies, state regulations, CQL standards, and all other regulatory bodies as required.
Complete and maintain all required training; this includes the Developmental Disabilities state requirements as well as Red River Human Services Foundation requirements.
Participate in other assignments/responsibilities as required.
Skills, Knowledge, and Abilities Required:
A minimum bachelor’s degree in social work or a human service-related bachelor's degree.
At least one year of experience working with people with developmental disabilities
The ability to communicate with all people (verbal and written) positively and effectively.
Good character and demonstration of the ability to establish positive, personal relationships with people we support.
The ability to assimilate and demonstrate proficiency in policies and procedures, personnel policies, regulations, and standards.
The ability to work as a team member and serve on a committee as assigned.
The ability to plan and organize.
The ability to meet personal vehicle insurance requirements.
