Job Description - Program Director of Case Management

Position Summary:


The Program Director of Case Management Services will be responsible for leading, managing, and overseeing all aspects of case management services. This role involves directing a team of case managers who provide trauma-informed, child-centered care to children and families. The Program Director will ensure that services are delivered within the guidelines of all governing entities while using Trust-Based Relational Intervention (TBRI) principles, to support healing and stability for children within programmatic services. The Program Director will play a critical role in shaping program strategy, ensuring quality care, and enhancing the overall impact of case management services.



Key Duties:


Leadership and Oversight of Case Management Services



  • Oversee the day-to-day operations of the case management program, ensuring the delivery of high-quality, child-centered services.

  • Lead and supervise a team of case managers, providing guidance, training, and support to ensure effective case planning and service delivery.



  • Maintain compliance with DFPS regulations, Texas Administrative Code, and foster care licensing requirements.



  • Monitor case management workflows, client progress, and outcomes, ensuring that services are appropriately tailored to the needs of children and families.


Staff Supervision and Development



  • Supervise, mentor, and support a team of case manager supervisors along with their teams, providing regular feedback and performance evaluations.

  • Ensure staff have the necessary skills and training to maintain compliance and provide trauma-informed care, including ongoing education on TBRI and related topics.

  • Promote a positive, collaborative work environment where team members are encouraged to share best practices and learn from one another.


Program Design and Implementation



  • Develop and implement case management protocols, policies, and procedures, ensuring they align with the organization’s mission and inline will all rules and regulations.

  • Work with the Chief Program Officer to ensure case management services align with the strategic goals of Divinity Family Services.

  • Continually assess and refine case management practices, ensuring they meet the evolving needs of children and families and adhere to all rules and regulations.


Client-Centered Care and Support



  • Provide oversight in the development of individualized service plans for children and families, ensuring a strengths-based, holistic approach that emphasizes relationship-building and emotional healing.

  • Ensure that all case managers are engaging with children and families in a manner that promotes safety, trust, and empowerment.

  • Act as a resource for case management supervisors in challenging situations, offering guidance and support when needed to address complex or sensitive cases.

  • Advocate for children and families within the system, ensuring they have access to appropriate services and resources to support their well-being.


Data Tracking, Evaluation, and Reporting



  • Oversee the tracking of client progress and case outcomes, ensuring that all case notes, assessments, and interventions are documented accurately and in a timely manner.

  • Collaborate with the Quality Assurance team to collect and analyze data on case management effectiveness and client outcomes.

  • Regularly report on program performance, including successes, challenges, and areas for improvement, to the Chief Program Officer.


Collaboration and Community Engagement



  • Foster relationships with external partners and community organizations to ensure clients have access to a broad network of services, including healthcare, education, and social services.

  • Work closely with other program leaders to ensure integrated care across services, particularly when clients are involved in multiple programs or services.

  • Represent Divinity Family Services in meetings, conferences, and community events to advocate for case management services and enhance the organization's presence and impact.


Compliance and Quality Assurance



  • Ensure that case management services comply with all relevant regulations, standards, and organizational policies.

  • Monitor and assess program compliance with licensing requirements, ensuring services are delivered in a manner that meets both state and federal standards.

  • Conduct periodic program audits to assess service delivery, identify gaps, and recommend improvements to ensure the highest quality of care.



Qualifications:


The employee must meet the minimum age requirements for full-time work in accordance with Texas child labor law and the federal fair labor standards act. This job description will act as an acknowledgment that the employee does meet the minimum age requirements set by state and federal labor guidelines. 




  • Bachelor’s degree with at least 5 years of work experience within child welfare.

  • Strong understanding of the rules and regulations CPA’s abide by.

  • Outstanding verbal and written skills, and experience working with other team members on all levels.

  • Supervisory or management experience preferred.

  • LCPAA credential preferred.

  • Preferred Bilingual (English and Spanish based on service area and service population).



Conditions of Employment


Employees must maintain a valid Texas driver’s license and motor vehicle insurance coverage as required by Texas law. In accordance with Texas law, Divinity Family Services (DFS) will conduct criminal background checks on all candidates and reserves the right to conduct random drug screenings during employment. Employees may be required to work weekends or evening hours as necessary to meet the needs of the organization. Employees are required to always keep a functioning cellular phone to ensure availability for on-call and after-hours duties.



The physical demands of the position may require employees to stand, walk, sit, use hands to finger, handle, or feel; reach with hands and arms; climb or balance; stoop, kneel, crouch, or crawl; and talk or hear. On rare occasions, employees may be required to lift 30 pounds.



In compliance with the Health Insurance Portability and Accountability Act (HIPAA), employees are required to adhere to all applicable federal and state laws, as well as DFS’s policies and procedures, regarding the safeguarding of confidential healthcare information.
