Job Description - Program Manager - Family Partner

General Description

To provide administration and oversight of Faison Center’s Family Partner residential program. The Family Partner

Program Manager’s primary responsibilities include: assessing each individual’s needs, writing support instructions

related to each individual’s person-centered outcomes, communicating with member’s of the individual’s support team,

training and supervising staff, scheduling supports and activities, and monitoring and ensuring compliance with all

federal and state laws, statutes, and regulations.

Essential Core Responsibilities

Case Management

  • Effectively and efficiently manages a caseload of up to 15 individuals (e.g., conducting assessments, writing Part 5 Plan for support, monitoring and evaluating progress, modifying care plans, training caregivers, writing progress reports, etc.) from intake to discharge.
  • Ensures that required documentation related to case management (e.g., incident reports, assessment reports, Part 5s, consents, quarterly reports, session notes, etc.) for assigned individuals is completed in a timely manner.
  • Submits documentation to state agencies (e.g., DBHDS, DMAS, DARS, etc.) and support coordinators in a timely manner.
  • Communicates progress towards individual program outcomes through the completion of quarterly reviews for assigned individuals and through correspondence with other members of the treatment team (e.g., individual, guardian, Case Manager, etc.)
  • Monitors progress on ISP outcomes and meeting with the clinical team to evaluate progress and modify outcomes and procedures at least quarterly.
  • Works collaboratively with other service providers
  • Actively participates in the ISP planning process by attending ISP team meetings, participating in SIS assessments, and contributing to the development of person-centered ISP outcomes and support activities.

Administrative Oversight, Performance Evaluation, and Regulatory Compliance

  • Communicates the expectations and responsibilities to the direct support professionals (DSPs) they supervise
  • Provides DSPs with guidance or takes remedial action to the extent necessary to ensure: a) provision of services and b) necessary documentation
  • Provides guidance to DSPs on identifying individual-specific changes that may indicate the need for an emergency response or team meeting (e.g., complex medical and / or behavioral health needs).
  • Completes scheduled and unannounced visits to licensed settings in order to facilitate and regulatory compliance.
  • Documentation.
  • Ensures that all Family Partner caregivers have completed and have met competencies for all required training.
  • Essential Personnel / On-call Requirements
  • Provide emergency support to Family Partner individuals as needed.

Requirements

Experience Requirements

  • One – two years of prior experience in a position of similar scope, preferred.
  • Ability to supervise and motivate a team of staff.
  • Ability to communicate through verbal and written mediums.
  • Ability to observe, ask questions, listen actively, and give and accept feedback effectively.
  • A high energy level, adaptability, assertiveness, patience and ability to work in a team environment.
  • Operation of a motor vehicle is required which includes proof of a valid driver’s license, an ability to maintain automobile liability insurance as required by state law in Virginia and an ability to qualify as a driver under Faison insurance guidelines.

Education Requirements

  • High school degree or equivalent required.
  • Bachelor’s degree preferred

Essential Physical Requirements

  • Lift 50 pounds.
  • Bend at all joints fluently.
  • Move in and out of and maintain a kneeling position for up to five minutes.
  • Reach in all directions.
  • Grasp using both hands. 
  • Move quickly in both confined and open spaces to avoid safety issues.
  • Run in order to retrieve an individual who is moving quickly away from staff.
  • Maintain balance while lowering to a half squat position.
