ESSENTIAL DUTIES AND RESPONSIBILITIES
· Receive project referrals and facilitate warm handoffs to assigned Community Health staff.
· Liaise with the community partners at Brooklyn hospitals and community-based agencies to assess the needs of the respective client populations and ensure a seamless referral process.
· Provides teaching and guidance to maternal and child health staff to improve quality and quantity of services provided.
· Evaluates and provides regular feedback on staff's performance during weekly supervision sessions.
· Reviews, evaluates, and interprets client records, vital statistics, and other data affecting client health and well-being to assess client needs and to develop client's care plan.
· Assists in planning educational programs for staff, related professional workers, and community groups to meet needs of personnel, clients and service providers.
· Provide behavioral health coordination and referrals to the Behavioral Health Team for those clients who have been identified as having behavioral health needs.
· Attends meetings, serves on committees, etc. and otherwise acts on behalf of project director in project director's absence.
REQUIRED EXPERIENCE AND SKILLS
· Licensed Master of Social Work with behavioral health experience and active license
· Successful supervisory experience
· Excellent organization and project management skills
· Excellent communication skills
· Excellent computer application skills, particularly Microsoft Outlook, Word and Excel
· Capacity and desire to work in a diverse, multicultural environment and community, with strong intention to eliminate the disparities affecting the black and brown communities in central Brooklyn
· Ability to build trusting relationships and work collaboratively in a dynamic, fast-paced environment
EXPERIENCE
· Licensed Clinical Social Worker with behavioral health experience and active license required (e.g LMSW)
· Experience working with maternal-child health populations and at-risk individuals
· Understanding of and/or working relationships with Brooklyn-based health and social service organizations
