Program Manager - Supported Housing & OMH Residential

The Residential Program Coordinator is responsible for the overall management and day-to-day operations of an OMH-funded supportive housing program serving individuals with serious mental health needs and/or histories of homelessness. The Manager provides leadership and supervision to program staff, ensures residents receive appropriate supportive services, oversees clinical and housing-related documentation, and maintains compliance with OMH regulations, contractual requirements, and agency policies.

The position requires strong leadership, clinical documentation, crisis-management, communication, and organizational skills, with a focus on promoting resident stability, independence, wellness, and successful community living.

Essential Functions:

• Oversee the daily operations of the supportive housing program and ensure effective delivery of services to residents.
• Ensure program activities are consistent with OMH regulations, contractual requirements, agency policies, and best practices.
• Monitor program performance, service delivery, resident outcomes, vacancies, incidents, and required program data. Including monitoring of the Single Point of Access system (SPOA) for referrals and update accordingly.
• Coordinate with landlords and Catholic Health attorneys for tenant leases.
• Submit monthly rent file and complete patient rental worksheets annually and as needed.
• Maintain a safe, supportive, trauma-informed, and respectful residential environment.
• Coordinate with agency leadership and other departments to address operational and resident-related needs.
• Supervise, support, and provide ongoing guidance to case manager and other program personnel.
• Conduct regular individual supervision, staff meetings, performance evaluations, and training.
• Monitor staff performance and documentation to ensure services are provided consistently and appropriately.
• Assist with staff recruitment, onboarding, training, and professional development.
• Provide guidance and support to staff managing complex resident situations and crises.  
• Oversee the delivery of individualized supportive services designed to promote housing stability, independent living, recovery, and community integration.
• Complete, review, and update OMH treatment plans for community residents, ensuring that plans accurately reflect residents' mental health needs, strengths, goals, progress, and service interventions.
• Collaborate with residents, case managers, clinical staff, and other service providers to develop individualized treatment goals, measurable objectives, and appropriate interventions.
• Monitor resident progress toward treatment-plan goals and revise plans as needed based on changes in residents' needs, functioning, or circumstances.
• Ensure treatment plans, progress notes, assessments, and related clinical documentation are completed accurately and within required timeframes.
• Ensure clinical and program documentation meets OMH, contractual, regulatory, and agency standards.
• Advocate for residents' needs and assist with connections to mental health services, healthcare, benefits, employment, education, and community resources.
• Promote resident choice, self-determination, recovery, independence, and quality of life.

Housing Stability

• Monitor residents' housing stability and address issues that may place tenancy at risk.
• Work collaboratively with residents and staff to address rent arrears, lease violations, behavioral concerns, conflicts, and other housing-related issues.
• Coordinate with landlords, property managers, housing providers, and community agencies regarding resident and property concerns.
• Support residents in developing skills necessary to maintain their housing and live successfully in the community.
• Assist with move-ins, transitions, transfers, and other housing-related processes as needed.
• Crisis Intervention and Risk Management
• Provide leadership and direction during resident crises and emergencies.
• Coordinate with mental health providers, hospitals, mobile crisis services, emergency services, and other community resources when appropriate.
• Ensure incidents are documented accurately and reported according to OMH and agency requirements.
• Identify potential risks to residents, staff, and the community and implement appropriate interventions.
• Participate in emergency response, safety planning, and follow-up after significant incidents.
• Compliance, Quality Assurance, and Documentation
• Ensure resident records and program documentation are accurate, complete, current, and maintained according to applicable requirements.
• Review treatment plans, progress notes, assessments, incident reports, and other required documentation for quality and compliance.
• Prepare for and participate in OMH reviews, audits, site visits, and other regulatory or contractual monitoring activities.
• Identify documentation or service-delivery deficiencies and implement corrective actions as necessary.
• Monitor program outcomes and participate in quality-improvement initiatives.
• Maintain confidentiality and comply with all applicable privacy and recordkeeping requirements.
• Responsible for ensuring timely reporting of work-related injuries, via incident/accident reports, to the Human Resources Department within 24 hours.
• Follows emergency / disaster preparedness protocols.         
• Remains flexible and available as it relates to personally providing coverage for a variety of emergencies, holiday, relief and weekend shift coverage, as needed.
• Follows agency policy in regards to attendance.
• Follows agency policy in regards to lateness.
• Complies with dress, appearance standards.
• Interrupts and reports suspected abuse to Administrator immediately.
• Keeps supervisor apprised of all significant issues.
• Employee conducts themselves in conformity with the HIPAA Compliance Program, applicable institutional policies and procedures for individual privacy and the Mercy Hospital Electronic Information Security Policies and Procedures.
• Job requires access to all of the individuals protected health information (PHI), in accordance with the Health Information Privacy and Accountability Act (HIPAA) regulations.
• Oversees performance improvement initiatives, understands current trends and demonstrates the ability to translate information into performance improvement reports.

Additional Responsibilities:

• Performs other incidental and related duties as required and assigned.
• Attends meetings and participates on committees as directed.
• Maintain required trainings as per agency policy.
• Provides supervisory “on call” coverage as indicated.
• Participates in educational programs, in-service meetings, workshops, conferences and
• committees as needed. Keeps abreast of current literature and professional trend.
• Required to notify management of arrest, criminal violation, field appearance ticket+/or bench
• warrant prior to next scheduled shift.
• Required to report suspensions/tickets received while driving agency vehicle and personal
• vehicle.
• Participates in Corporate Compliance Program by receiving formal in-service education,
• obtaining a copy of the Compliance Assurance Program Handbook and attesting to abide by   
• the Standards of conduct outlined in the handbook.

Education and Qualifications:

• Master’s degree in Human Services preferred. Master’s degree required.
• A minimum of 2 years of experience with psychiatric population required.
• Supervisory experience preferred.
• Experience developing, reviewing, or maintaining treatment plans and clinical documentation.
• Knowledge of OMH programs, supportive housing, community-based behavioral health    services, and housing-stability practices preferred.
• Strong understanding of person-centered, recovery-oriented, trauma-informed, and culturally responsive approaches.
• Excellent written, verbal, organizational, leadership, and interpersonal skills.
• Ability to manage multiple priorities, respond effectively to crises, and work collaboratively with residents, staff, providers, landlords, and community partners.
• Strong computer and electronic-record documentation skills required.
• Maintain valid NYS driver’s license within agency criteria.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience.
