Job Description - Program Supervisor II-Victoria-ICF/DD


Job Summary: The Program Supervisor II (PS) is responsible for the oversight, leadership, and daily operations within an ICF setting.  The PS is responsible for training employees, ensuring the client’s care plans are followed, and overall budget management.  The PS is responsible for development, implementation, and monitoring of the clients’ individual program plans.

Mount Olivet Rolling Acres - MORA – is a diverse and inclusive nonprofit has supported adults, seniors and children with developmental disabilities, behavioral health needs as well as age-related and chronic medical conditions for over 50 years in the Twin Cities metro.

MORA provides an array of services: Adult Day Services, Contracted Case Management, Community Supports, over 30 Residential Homes, Respite, Therapeutic Recreation, Training for Guardians & Families, and a Discover Summer Daytime Programming for Youth with Disabilities.  Our Metro Crisis Coordination Program (MCCP) offers a safety net of crucial and cost-effective services for adults and children.

Hourly Base Pay:  $26.00-$30.00  per hour   DOQ  
Weekend differential +$2.00 hour for hours between Fri 10pm and Sun 10pm
Overnight differential +$1.00 hour for hours between 10pm and 6am
($12.13 per hour sleep hours only when assigned)     

$1,500 retention bonus for full-time paid out over 1 year.

Schedule: 
Monday-Friday 40 hours FLEX Hours

Supervisor on call rotation 

 

Key Duties and Responsibilities:

  • Demonstrates an understanding of the person as a unique individual and how to write and implement programs in accordance with the Support Plan (SP), Health Care Plan (HCP), and other supporting 245D documentation
  • Provides person-centered services demonstrated through practice, action, and language
  • Communicates effectively with others at work verbally and in writing, using the primary spoken language of clients in the home
  • Maintains a working knowledge of person(s) history, skills, areas of vulnerabilities, habilitation plans, and emergency procedures. 
  • Develop, implement, and evaluate as needed the individual program plans and all other required documents and assessments for clients
  • Responsible for the overall 245D, Minnesota Department of Health (MDH), and Minnesota State Fire Code licensing compliance
  • Responsible for the overall oversight of household communications (email, phone, communication book, house calendar)
  • Responsible for the overall program budget, petty cash, and oversight of client account
  • Completes quality improvement tool (Radar) unless delegated where appropriate
  • Completes MORA Program Supervisor Scorecard monthly and ensures completion of the scorecard for individuals on the team
  • Supervise, train, support and evaluate staff
  • Liaison with day programs, guardians, and case managers. Works closely with the IDT to ensure continuity of care between providers
  • Work harmoniously with and shows respect for employees, supervisors, vendors, county partners, and clients
  • Checks their MORA email each shift, utilizes SharePoint and Teams as necessary to complete documentation and remain current on MORA and client information
  • Responsible for the overall coordination and execution medical and psychological appointments
  • Lead interdisciplinary team planning and meetings along with all program staff meetings
  • Provide leadership and shift supervision
  • Participate, attend and or chair committees, teams and or focus groups as needed as assigned
  • Provide oversight for the planning and implementation of recreation, leisure, and community activities, including establishing and maintaining community relationship and facilitating friendships
  • Ensures adequate supervision of clients, including remaining on shift until a replacement employee arrives
  • Works closely with the Program Coordinator and Employee Scheduler to ensure that all shifts including day program closures and PPL requests are placed on the WorkSchedule and are filled
  • Present at MORA special events whenever able
  • Present on-site for majority of working hours
  • Work direct care shifts as needed and assist with daily cares, ADL’s, behavioral and medical needs of clients
  • Remains awake at all times unless working an assigned sleeping shift
  • Provide advocacy and assist in emergency situations, including emergency room or urgent care visits
  • Responsible for the overall management of the home in conjunction with the Program Coordinator.  If there is not a Program Coordinator for the site, essential job duties for that position are the responsibility of the PS unless delegated.
  • Completes all mandatory training either determined by MORA or 245D regulations
  • Reads and follows company policies and procedures
  • Perform other duties as assigned
Positions that include flex hours:
  • Flexible hours are to be used based on the client needs, including but not limited to; client annual meetings, medical appointments, and day program closures.
  • Flex time is also used to provide oversight and management of administrative tasks within the home, including but not limited to; assisting with training, house schedules as needed, onboarding employees, 245D licensing visits, 245D paperwork, evaluation of employee’s performance and care for clients, MORA Scorecard, and other pertinent matters within the home.

Desirable Qualifications:

  • Three or more years of experience working with persons with developmental disabilities
  • Proven leadership skills in disability services, prefer supervision experience
  • Proven skills in behavior management techniques and skill development.
  • Strong ability to work with and support a team in understanding client healthcare needs, management of a team and client care plans, and working closely with the IDT and MORA Team (PD, PC, MC, Nurse)
  • CPR and First Aid Certified

Required Qualifications:

  • Bachelor’s degree or higher in the Human Services field, including, but not limited to: Social Work, Sociology, Special Education, Rehabilitation, Counseling or Psychology
  • QDDP/QIDP status per Department of Human Services
  • Must pass Department of Human Services background check and maintain qualified status
  • Must not be excluded from working in government health programs by the Office of Inspector General
  • Must be able to successfully complete company’s physical job demands document.
  • have a valid current driver’s license and must have vehicle insurance as required by state law
  • Successful completion of Medication Administration class, regularly passes medications using correct procedure without error
  • Ability to use the computer and do so in alignment with client needs for healthcare and overall care
