Schedule Details: Will work primarily at the Pewaukee Cancer Center. No weekends.

Position Summary

We are seeking a compassionate and highly organized Oncology Nurse Navigator to guide patients through the cancer care continuum. This role serves as a key point of contact for patients, families, and the multidisciplinary care team, ensuring timely access to care, coordination of services, and a seamless patient experience.

The Oncology Nurse Navigator provides clinical expertise, education, and support from diagnosis through treatment and survivorship, while helping to remove barriers to care.

Key Responsibilities

• Coordinate and ensure seamless, evidence-based care across the oncology continuum


• Conduct comprehensive assessments and collaborate with the multidisciplinary team to develop individualized care plans


• Provide patient and family education, support, and advocacy, addressing clinical and psychosocial needs


• Identify and address barriers to care to promote timely access and adherence to treatment


• Develop and deliver education materials and outreach programs for patients, staff, and the community


• Participate in quality improvement initiatives and maintain clinical expertise to support optimal patient outcomes
