Job Title: Provider Relations Manager (Outside Account Executive)
Location: Field-based (NYC – All Boroughs & Westchester) | Hybrid | HQ in Astoria, Queens
Compensation: $80,000–$120,000 base + uncapped variable compensation
On-Target Earnings (OTE): $135,000–$175,000 (first 12 months)

Care2U is redefining healthcare delivery by offering hospital-level, emergency medicine-led services directly to patients’ homes. We partner with health plans, ACOs, hospitals, home care agencies, and provider groups across New York to reduce preventable emergency department utilization and improve value-based care outcomes. Our care model serves patients in all five NYC boroughs, Westchester, and Long Island.

About the Role

We’re looking for a tenacious and relationship-driven Provider Relations Manager to serve as the face of Care2U across a defined geographic territory. You'll be assigned a book of warm PCP and specialist practices tied to corporate partnerships. Your goal is to activate these practices, and drive consistent, high-quality referral volume. You can also expect to have providers in your workbook that include hospital discharge planners, home care agencies, and senior living facilities. An activated practice is one that sends at least one referral that results in a completed patient visit. Once ramped (within 6 months), you’ll be expected to generate 300+ referrals per month from your book of assigned practices.

Key Responsibilities

  • Own Your Territory: Manage a portfolio of warm accounts within a specific NYC or Westchester territory, regularly visiting practices and building strong provider relationships.
  • Drive Referral Growth: Activate and penetrate assigned practices by educating, engaging, and supporting staff and providers—ensuring consistent monthly referral volume.
  • Penetrate Practices: Ensure all providers within a group are referring regularly, not just one champion.
  • Hit Referral Targets: Ramp to 300+ monthly patient referrals by month 6 through persistent follow-up, trust-building, and thoughtful territory planning.
  • Deliver Practice Education: Clearly communicate Care2U’s high-acuity home care model, outcomes, and operational process to simplify referrals.
  • Track Performance: Log activity and referral metrics in CRM and provide feedback to internal teams for workflow and experience improvement.
  • Collaborate Across Teams: Work closely with intake, clinical, and operations teams to ensure white-glove service to patients and referring offices.
  • Network: Attend relevant networking events and community forums in the eldercare, home care, and hospital ecosystems to build relationships and grow referral pipelines.

Preferred Qualifications

  • Familiarity with urgent care, telehealth, or home-based care models
  • Relationships with providers or IPAs in NYC or Westchester
  • Bilingual (Spanish or other languages) is a plus
  • Understanding of value-based care or risk-bearing entities

Requirements


Qualifications

  • 3–5+ years of success in field sales, provider relations, or physician engagement—preferably in a healthcare, CHAA, Hospitals or home-based care setting.
  • Proven ability to manage a large territory and high account volume.
  • Comfortable in a quota-carrying, performance-driven role.
  • Strong communication and relationship-building skills.
  • Organized, self-directed, and resourceful.
  • Experience using CRM tools such as Salesforce or Zoho.
  • Valid driver’s license and willingness to travel within assigned geography.
