The mission of Lyon-Martin Community Health Services is to provide high quality, compassionate and trauma-informed medical, gynecological, and mental health care services to trans, non-binary, gender non-conforming, and intersex communities and cis-gender women with specific sensitivity to LGBQA+ sexual orientation, disability, size, race, ethnicity, and language, regardless of immigration status or ability to pay.

Job Summary
The Psychiatric Nurse Practitioner (Psych NP) is primarily responsible for providing clinically appropriate psychiatric care services to Lyon-Martin clients with a wide array of psychiatric needs, including psychiatric medication management, substance use disorder treatment, crisis intervention, de-escalation and safety planning, and leveraging appropriate internal/external referrals. The Psych NP is expected to maintain a high degree of professional competence, the highest possible ethical standards, and to work collaboratively as part of an integrated health care team. This position may be required to work some evenings.
Key Responsibilities
Clinical Care Responsibilities
Under the supervision of the Supervising Psychiatrist, evaluates, diagnoses, and treats mental health and psychiatric conditions within individual patient sessions.
Takes thorough medical and psychosocial histories, differential diagnosis, orders and interprets lab tests and diagnostics studies when needed.
Provides specialized substance use disorder treatment to those with complex psychiatric needs.
Work collaboratively as part of an interdisciplinary team to coordinate and integrate services for clients with complex psychiatric needs.
Demonstrates strong clinical knowledge of and commitment to healthcare for trans, gender non-conforming and intersex people, LGBQA+ communities, cis-gender women, and Black and Brown communities.
Provides back-up crisis intervention support during clinic hours of operation, monitoring the crisis triage Team chat, responding quickly and intervening with in mental health emergencies or high emotional distress during clinic, de-escalating, doing safety planning with clients presenting with suicidality, as appropriate.
Utilizes Mobile Crisis, Dore Urgent Care as appropriate.
Provides psychiatric consultation and clinical trainings to other providers and staff as needed.
Provides consultation support to on-call providers for After Hours crisis calls for clients who are in extreme mental distress including those experiencing suicidality or potentially using lethal drugs alone.
Uses a harm reduction model and links clients into complimentary care including but not limited to therapy, case management, group support, pain management, Traditional Chinese Medicine, Physical Therapy, or higher levels of care as appropriate.
Provide short-term

counseling/psychotherapy

to those engaged in psych care and waiting to access a long-term therapist or higher levels of care.
Work with the Mental Health Care Coordinator to ensure coordination of referrals to inpatient, residential, or intensive outpatient care facilities in-network with insurance whenever possible are successful and efficient.
Performance and Quality Improvement Responsibilities:
Meets benchmarks for applicable visit numbers and quality indicators as assigned (expectation is 7 scheduled visits per half day unless longer appointments are needed for full

evaluation/intake).
Documents visit notes and client interaction in an accurate, detailed, and timely manner (ideally same day, maximum within 72 hours). Responds to in basket tasks within 72 hours (urgent ones same day).
Maintains appropriate boundaries with clients, especially when experiencing transference and

countertransference.
Follows through on commitments with clients and colleagues.
Attends and participates in weekly team meetings, weekly one-on-one supervision meetings with Supervising Psychiatrist, monthly Collaborative Care meetings, and monthly All Staff meetings.
Maintains at least two 8 hour in-person clinical days per week to accommodate clients who do not have access to the technology required for phone or telehealth appointments.
Meets benchmarks for all contract and grant deliverables.
Strong initiative and enthusiasm and willingness to pitch in whenever needed.
Gets along with colleagues, assists in continuous quality improvement through on-going problem/needs identification and problem solving and, as requested, acts as a champion of change and a leader for improvement activities.

Education, Knowledge, Skills:
Graduation from an accredited Psychiatric Nursing school.
Licensed in California as a Psych NP.
At least 2 years of experience safety planning with clients who are actively suicidal.
At least 2 years of experience providing nursing care in a community clinic setting.
Extensive knowledge of Bay Area mental health resources.
Excellent de-escalation and non-violent communication skills.
Experience mediating using a restorative justice lens.
Experience counseling those with substance use disorder using a harm reduction approach, especially with clients actively using drugs.
Culturally sensitive/humble and able to work with a diverse population.
Strong proficiency with Microsoft Office applications, specifically Word, Outlook, internet applications, zoom, computer technology, and phone use
Experience working with lesbian, bisexual, transgender, gender non-conforming, non-binary, and intersex communities.
Experience working with populations with varying disabilities, racial, ethnic and cultural backgrounds and economic statuses.
Experience working with people who do sex work, those experiencing homelessness, people who have challenges with severe mental illness, people who use drugs, those of with a history of incarceration, people of varying ages.
An interest in and commitment to accessible gender affirming health care, abortion and primary care for marginalized populations.
Desired:
Bilingual Spanish/English fluency; other bilingual or multilingual fluency
