Job Description - Psychiatric Provider

Description

JOB SUMMARY

Develop and maintain an outpatient integrated behavioral health practice within assigned ambulatory medical clinics of Appalachian Mountain Health (AMH). Conduct clinical evaluations which produce diagnostic findings to identify appropriate clinical intervention based on patient’s symptoms, history, and health goals. Create a patient specific treatment plan for psychiatric medication management. Operate within a fully integrated model of health care delivery by collaborating and coordinating with physicians, advanced practitioners, behavioral health providers, and all levels of clinical staff through a shared patient population to promote overall whole person care.

KEY ACCOUNTABILITIES

Conducts psychiatric diagnostic evaluations in person or by telemedicine.

Performance Criteria:

  1. Performs Bio-psychosocial assessments
  2. Promotes and reviews the screening of patients for behavioral health concerns.
  3. Performs individual and family assessments through record review; provider consults, interviews of patients, caregivers and others as needed, observation, data collection, genograms, and administration of appropriate assessment tools.
  4. Performs comprehensive assessment for substance use disorder patients being considered for Medication Assisted Treatment for Opioid or Alcohol Use Disorder
  5. Document within the patients Electronic Health Record (EHR) a thorough report of the diagnostic findings and appropriate medication, and treatment recommendations.
  6. Demonstrates an ability to evaluate and triage a wide range of behavioral conditions within in the context of a fast paced, medical care practice.
  7. Performs patient risk assessments as needed based on patient specific risks, both mental health risks and patient’s specific health and metabolic risks.

Delivers quality, evidence based behavioral health treatment in an integrated care model in person or by telemedicine.

Performance Criteria:

  1. Prescribes evidence-based appropriate psychiatric medications based on patients various risk factors and treatment goals.
  2. Provides appropriate patient education concerning the patient’s diagnosis and treatment.
  3. Monitors patient progress related to treatment, and adjusts treatment accordingly
  4. Recommends evidence based behavioral therapy services and collaborates with other internal providers to ensure access to those services including:
  5. Individual, family, and group therapy
  6. Treatment for substance use disorders
  7. Motivational Interviewing for chronic disease self-management, substance use disorders, and mental illness
  8. Episodic targeted brief interventions and psycho –education for depression, anxiety, and adjustment disorders.
  9. Communicates effectively with people of all ages who may have complex medical, developmental and behavioral conditions, and their caregivers.
  10. Supports practice by establishing outcome-based treatment plans, providing brief strategic interventions, and evaluating progress toward stated goals.
  11. Works effectively with practice staff and care management to facilitate referrals to community mental health services when appropriate.

Maintains quality of clinical services provided.

Performance Criteria:

  1. Maintains accurate documentation within the Electronic Health Record (EHR), according to institutional, professional and insurance compliance standards, within 24 – 72 hours of clinical encounters
  2. Adheres to schedule by providing services within 30 – 60 minutes of scheduled appointment
  3. Keeps abreast of the most recent and accurate applicable research, patient care practices, coding and billing information.
  4. Releases patient information by adhering strictly to legal, ethical, hospital and department guidelines.
  5. Maintains organizational and departmental productivity standards of face-to-face patient contact.
  6. Monitors quality indicators and participates in quality improvement and program development initiatives as directed. Participates in data collection and compilation.

Supports department by pursuing personal, professional, institutional and community development.

Performance Criteria:

  1. Maintains appropriate state clinical license in good standing.
  2. Participates in an appropriate level of ongoing clinical supervision.
  3. Participates in professional associations and networking as appropriate.
  4. Gathers updated knowledge regularly by participating approved pertinent continuing education.
  5. Supports department by contributing to staff development and the enhancement of quality patient care.
  6. Supports department by participating in approved community collaborative initiatives.
  7. Participates in research projects and grant opportunities as directed and appropriate.

Supports an integrated model of care by completing other duties as assigned or requested.

Performance Criteria:

  1. Collaborates with medical provider colleagues to develop and maintain strong patient centered interdisciplinary teams.
  2. Attends departmental meetings and appropriate AMH meetings as requested.
  3. Participates in precepting and supervising learners as directed and appropriate, including supervision of integrated care behavioral health intern as needed.
  4. Supports the program by completing other duties as assigned or requested.

Requirements

EDUCATION/EXPERIENCE

  • Certification as Nurse Practitioner or Physician Assistant: Additional certification as PMHNP or PA-C psych
  • PMHNP or PA-C
  • Full licensure with a minimum of one-year postgraduate experience  

Preferred Qualifications

  • Three years postgraduate experience with one-year experience providing psychiatric care in a medical setting. Familiarity with outpatient billing/coding procedures, NC Medicaid guidelines, local MH/SA/IDD resources. Depending on practice location- experience working with children, adolescents, adults, and geriatric populations. Spanish speaking a plus.
