Psychotherapist/Clinical Social Worker - Health Center

POSITION SUMMARY:

Huckleberry Youth Programs’ mission is to work in solidarity with young people and their families to advance personal growth and social equity. We are a community-based organization providing a spectrum of services to youth ages 12–24 and their families in San Francisco and Marin counties, with programming focused on safety, health, justice, and education.

The Counseling Program in San Francisco offers individual and family therapy, case management, groups, and behavioral health classes. We strive to deliver culturally responsive, high-quality services in English and Spanish. Our therapists are embedded across multiple program sites—including the Huckleberry Youth Health Center, Huckleberry House youth shelter, the Huckleberry Advocacy and Response Team (HART), and the Community Assessment and Referral Center (CARC)—and may also be placed at schools or other community locations. Clinicians receive weekly group supervision and staff support, and maintain caseloads that allow them to book at least twenty clients per week.

The Outpatient Psychotherapist/Clinical Social Worker will be embedded at the Huckleberry Youth Health Center (555 Cole Street, San Francisco). This position primarily provides family and individual therapy to clients with Medi-Cal coverage, while working collaboratively with the Client Services team (Case Management and Therapy), Create 2 Activate Project Coordinator, Health Education team, and the Cole Street Clinic (operated in partnership with the San Francisco Department of Public Health). Together, these teams provide free and confidential wraparound services for young people ages 12–24, including basic needs support, Narcan distribution, and other harm-reduction resources.

Essential Functions

  • Provide individual therapy to a range of clients ages (12-24) and Medi-Cal funded clients (ages 12–21).
  
  
  • Conduct comprehensive assessments with youth, including the CANS, TESI, and Huckleberry Youth Health Center’s Assessment for Services.
    
  
  
  • Demonstrate proficiency in biopsychosocial assessments, diagnostic formulation, treatment planning, and evidence-based interventions to bring about therapeutic change.
    
  
  
  • Ensure youth are connected to supportive services as appropriate, including psychiatric care, substance use disorder treatment, case management, and primary medical care.
    
  
  
  • Obtain client feedback using the PCOMS (Partners for Change Outcome Rating Scale) and required surveys (San Francisco County BHS and HYHC-specific).
    
    
  
  
  
  


• Community Outreach & Collaboration
  
  
  
  
  
  • Maintain quarterly outreach to youth-facing spaces, with a focus on reaching Medi-Cal–eligible youth and communities needing support in connecting to Medi-Cal services.
  
  
  • Conduct training and provide consultation for Huckleberry staff
  
  
  • Reduce barriers to accessing mental health care by demonstrating flexibility in service location and approach.

    • Monthly Health Center All-Site meetings
    
    
    • Bi-weekly HYHC team meetings
    
    
    • Weekly clinical supervision with the Clinical Director

    • Embedded therapist meetings, group supervision, and agency-wide meetings
      
      
    
    
    
    
  
  
  • Assist youth with navigating the Health Center by answering phones, logging clients in, and supporting service access as needed.
    
  
  
  • Collaborate closely with the Client Services team, Health Education staff, Create 2 Activate Project Coordinator, and the Cole Street Clinic team.

WORK ENVIRONMENT:

This position operates out of the Huckleberry Youth Health Center and may include travel to other Huckleberry sites or community locations for service delivery and coordination. This role routinely includes the use of standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.The Huckleberry Youth Health Center is located in San Francisco.

PHYSICAL DEMANDS:

The employee must be able to lift and/or carry up to 10 pounds unassisted on an occasional basis. While performing the duties of this job, the employee is regularly required to talk, hear and sit. The employee is required to stand, walk, climb stairs and use hands to find, handle, or feel objects, tools or controls. 

REQUIRED EDUCATION AND EXPERIENCE:

1. A master’s degree in social work or counseling

2. A clinical license or registered with BBS in pursuit of license 

PREFERRED EDUCATION AND EXPERIENCE:

1. Experience providing services to adolescents or Transitional Age Youth (TAY) 

2. Experience providing individual therapy

3. Ability to provide services in Spanish, Cantonese or other San Francisco  Threshold languages is strongly preferred 

4. Experience with Medi-Cal systems and documentation, use of  EPIC is strongly preferred 

5. Experience, awareness and knowledge of cultures of San Francisco youth and families and demonstration of cultural sensitivity and responsiveness

6. Experience with telehealth and/or other non-traditional modalities.  

ADDITIONAL ELIGIBILITY QUALIFICATIONS:

1. Recent TB clearance


2. Valid CA driver’s license and clear DMV record


3. Have or will complete first aid/cpr training


4. Fully vaccinated for COVID 19
