Job Description - Quality and Education Coordinator


Sangre de Cristo Community Care is seeking a detail-driven, self-motivated Quality and Education Coordinator to support our hospice and home health programs. This role is essential to ensuring regulatory compliance, driving quality improvement, and coordinating staff education.

The ideal candidate is highly organized, tech-savvy, and analytical, with a strong eye for accuracy and a passion for supporting excellence in patient care. If you enjoy working independently, managing data, and helping teams stay compliant and well-trained, this may be the role for you.

This is a full-time, benefited position, working Monday–Friday, 8:00 AM–5:00 PM, with a minimum starting wage of $18.00/hour.

Key Responsibilities


Quality & Compliance

  • Conduct daily, weekly, monthly, and annual reviews of quality data to ensure accuracy and completeness
  • Upload required quality metrics to CMS and state portals and verify successful submission
  • Generate, format, and distribute quality and performance reports
  • Create data visualizations using tables and graphs
  • Support quality improvement initiatives through data collection, tracking, and reporting
  • Maintain documentation to support compliance and audit readiness

Education & Training

  • Coordinate new employee orientation and ongoing education programs
  • Assign, track, and monitor training modules through the Learning Management System (LMS)
  • Monitor completion of staff competencies, annual education, flu vaccinations, and TB screenings
  • Send reminders and follow up on outstanding training or compliance items 

Administrative Support

  • Schedule quality and education meetings and support meeting logistics
  • Provide general administrative assistance and occasional front desk coverage as needed

Requirements

 

  • Minimum 1 year of experience in an administrative assistant or similar role
  • Experience auditing medical charting or medical records preferred
  • Strong attention to detail with the ability to identify variances and trends
  • Ability to work independently and manage multiple priorities
  • Advanced computer proficiency, including MS Office, Excel, and electronic medical record systems
  • Typing speed of 36 WPM or higher
  • Excellent written and verbal communication skills
  • Ability to read, interpret, and apply regulations, policies, and procedures
  • Working knowledge of Medicare Conditions of Participation for Hospice (42 CFR 418) preferred
  • Proof of TB testing (PPD and/or CXR) upon hire and annually thereafter
  • Proof of annual Influenza and COVID-19 vaccinations
  • Valid Colorado driver’s license with acceptable driving record
  • Must maintain required auto insurance and reliable transportation
