Quality Improvement RN

Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness is an independent, non-profit healthcare provider licensed in 11 counties in northwest Illinois. We have a state-of-the-art facility and the first free standing in-patient Hospice Home in Illinois. We have a 35 year history for providing compassionate professional care to terminally ill patients and their families. We are state licensed and Medicare/Medicaid certified.

We are currently seeking a full-time Quality Improvement RN to join our Quality, Compliance, and Risk Management team. This role will co-lead QAPI initiatives, serve as the clinical lead for CHAP accreditation readiness, and collaborate on policy development and process improvement. The Quality Improvement RN will also be responsible for educating clinical staff on documentation standards and infection control protocols, conducting audits, and supporting regulatory compliance. This position works Monday -Friday 8:00am to 4:30pm. 

Illinois Registered Nurse with a minimum of two-year experience in either home health or nursing home/long term care. Previous Hospice experience helpful.
