Sage Group HealthCare Staffing is seeking Home Health RN for a / contract position in St. Johnsbury, VT. Accepting both local and traveler candidates.

Position Details:

Position Name: RN - Home Health

Location: St. Johnsbury, Vermont 

Specialty: Home Health 

Start Date: ASAP

Employment Type: Full-time, Travel, Contract, Temporary

Contract Length: 13 weeks

Pay Rate: Range $2900 to $3000 Weekly Gross pay based on a 40-hour work week (Includes non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)

Mileage Reimbursement: $0.60/mile for distance covered between patient home appointments

Unit: Home or Community 

Shifts available: Days



Shift timings: 8 Hours; Days, 8a – 5.00p





Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri with rotating weekends. No call 

Position Requirements:



  • Registered Nurse: 1-2 years - Required

  • Home Health Nursing experience: 1-2 years – Required

  • EMR HCHB experience  - Preferred 

  • Must have valid Driver’s License and Personal Auto Insurance

Education, License and Certification Requirements:



  • ASN or an equivalent – Required

  • BSN or an equivalent – Highly preferred

  • RN License for State/s applying or Compact - Required

  • BLS Certification from AHA or American Red Cross - Required

Job Description:



  • RN Case Managers screen, evaluate, re-evaluate, and treat patients as well as coordinate and communicate treatment activities with the interdisciplinary team members and the patient.

  • Develops a care plan in collaboration with the patient's physician, the patient, and their family, or the primary caregivers upon admission. Periodically reviews the patient care plan and recertifies the patient care plan as appropriate.

  • Coordinate the patient's care plan through interdisciplinary communication and case conferences, as appropriate. Advise other disciplines of discharges in a timely manner.

  • Provide ongoing assessment of the patient's clinical, functional, and emotional status to identify problems and/or progress toward desired goals.

  • Communicate significant changes to the patient's physician and colleagues.

  • Document evaluations, treatment goals, and objectives.

  • Teach the patient and caregiver self-care techniques as appropriate.

  • Provide medication, diet, and other instructions as ordered by the physician and utilizes opportunities for health counseling with patients and caregivers
