Purpose of Position

The primary purpose of the Resident Care Coordinator (RCC) is to provide a wide variety of direct care to the residents and report to the Director of Nursing Services to ensure that the highest degree of quality care is always maintained.  The RCC is responsible for training of non-licensed clinical staff (Caregiver, Certified Nursing Assistant, and/or Medication Technician).  The RCC provides and models high standards of customer service by demonstrating the following: taking ownership of each and every interaction, communicating with a smile and accurately identifying customer needs and following through on requests, maintaining a clean work environment, and providing efficient service recovery by working quickly to resolve resident concerns. 

Essential Functions

• Coordinates in services and assist with ongoing education of non-licensed clinical staff.


• Participates in interviewing process for new staff.


• Creates and maintains the schedule for non-licensed clinical staff in conjunction with the Director of Nursing.


• Trains and mentors new clinical staff.


• Assists in auditing care tasks and electronic documentation.


• Participates with Director of Nursing and Executive Director in Care Conferences as appropriate.


• Responsible for coordination and oversight of non-licensed clinical staff onboarding and training experience.


• Assists Director of Nursing with day-to-day communications.


• Assists with gathering annual/quarterly/change in condition documentation.


• Ensures care are being performed up to The Carriages standards.


• Involved in participating in department meetings as assigned.


• Always maintain professional behavior while providing a wide variety of direct care to residents in a manner which ensures dignity and respect for those involved.


• Measures, records, and tracks vital signs assigned by the Director of Nursing.


• Clearly communicates important updates and information from nursing leadership within the nursing department.


• Maintains the confidentiality of all resident care information including protected health information.


• Report known or suspected incidents of unauthorized disclosure of such information to direct supervisor or Executive Director within 24 hours.


• Informs appropriate staff regarding status of resident emergencies and hospitalizations. 


• Reports pertinent observations regarding residents to Director of Nursing and/or Executive Director.


• Provides support to the department by being an active team member, contributing to and leading by example utilizing the teamwork concept. 


• Possess the ability to interact and communicate professionally with personnel, residents, family members, visitors, government agencies/personnel, and the public and demonstrates this daily.


• Provides services by being on the job, on time, and as scheduled.


• Self-motivated and driven by a can-do attitude which creates and fosters a positive work experience for self and others. 


• Flexible to be scheduled and help on various shifts including weekends and holidays.


• Performs all other duties as assigned by Director of Nursing and/or Executive Director.

Physical Requirements and Work Environment

While performing the duties of this job, the employee is required to frequently walk and stand for long periods of time, and frequently stoops, kneels, crouches or crawls, pushes, pulls, lifts, grasps, and feels.  The employee frequently is required to use manual and finger dexterity and eye-hand coordination when handling equipment.  Requires corrected vision and hearing within normal range.  The employee must occasionally lift and/or move up to 35 pounds.  Typical healthcare and residential environment.  The noise level is normally within a normal range.  Occupational exposure to blood, body fluids, communicable diseases or other potentially infectious substances (Category III).  Work schedule may include working day, evening, night shifts, weekends, and rotating holidays. 

Education and Experience

High School diploma or general education degree (GED).  Current CNA certification, Medical Assistant or Medication Technician certification required.  Current CPR certification is preferred.  Minimum of two years’ experience in a hospital, nursing care facility, or other related health care facility is preferred.  Must be able to read and write legibly in English.  Ability to speak English effectively to individuals, customers, or employees of the organization.  Must be able to comprehend and perform oral and written instructions and procedures.  Possess basic personal computer skills and ability to utilize various software programs.  Experience working with the elderly population is preferred.
