Job Title: Re-Entry Case Manager, LEAP (Linking Employment Activities Pre-release) Program


Location: Cape Girardeau, MO 63703


Reports to: Program Manager, LEAP Program


Job Overview:


The Re-Entry Case Manager for the LEAP (Linking Employment Activities Pre-release) Program provides critical support to individuals transitioning from incarceration to the community, helping them address barriers to employment and successful reintegration. This role involves direct engagement with participants both pre-release and post-release, focusing on case management, goal setting, and connecting participants to essential resources. The Re-Entry Case Manager will work closely with program staff, correctional facilities, community partners, and employers to create a comprehensive support network that promotes participant success.


Key Responsibilities:



  • Participant Assessment and Case Planning

    • Conduct intake assessments to evaluate participants’ needs, strengths, and barriers, including housing, transportation, education, mental health, and substance abuse.

    • Develop individualized re-entry plans with participants, setting measurable goals related to employment, housing, family reunification, and other key areas.

    • Adjust and update plans as participants progress, ensuring a flexible and responsive approach to changing needs.



  • Pre-release Support and Preparation

    • Provide pre-release counseling focused on addressing concerns related to reintegration, such as social support, employment readiness, and community resources.

    • Collaborate with correctional facility staff to organize pre-release workshops and trainings that cover life skills, job readiness, financial literacy, and personal development.

    • Help participants gather necessary documentation, such as IDs, social security cards, and work permits, to ensure a smoother transition upon release.



  • Post-release Case Management and Support

    • Conduct regular follow-up meetings with participants after release to monitor progress, offer support, and adjust case plans as necessary.

    • Assist participants in navigating post-release challenges, including finding stable housing, accessing health services, and meeting probation or parole requirements.

    • Connect participants to job opportunities, educational programs, and community resources, providing ongoing support for job retention and career advancement.



  • Community and Partner Engagement

    • Develop and maintain relationships with community organizations, social service providers, housing agencies, and treatment centers to create a robust referral network.

    • Work closely with employment specialists to match participants with suitable job opportunities and address any barriers to employment.

    • Represent the LEAP Program at community events, meetings, and collaborative initiatives focused on supporting justice-involved individuals.



  • Documentation and Reporting

    • Maintain accurate and up-to-date case files and records of participant interactions, progress, and outcomes.

    • Track key metrics, including participant engagement, job placements, recidivism, and goal attainment, for program reporting and evaluation.

    • Prepare reports on caseloads, program challenges, and participant success stories for internal and external stakeholders.




Qualifications:



  • Education and Experience:

    • Bachelor’s degree in social work, psychology, criminal justice, or a related field.

    • Minimum of 2-3 years of experience in case management, re-entry services, or a similar role, ideally with justice-involved individuals or other vulnerable populations.



  • Skills and Competencies:

    • Strong knowledge of re-entry challenges and resources, including housing, employment, substance abuse treatment, and mental health services.

    • Proficiency in motivational interviewing, trauma-informed care, or other counseling techniques relevant to justice-involved populations.

    • Excellent organizational and time-management skills, with the ability to manage multiple cases effectively.

    • Strong interpersonal and communication skills, with the ability to establish trust and rapport with participants.

    • Familiarity with data tracking and case management software, as well as Microsoft Office Suite.




Preferred Qualifications:



  • Knowledge of local service providers, re-entry resources, and employment opportunities for formerly incarcerated individuals.

  • Experience with restorative justice, conflict resolution, or other re-entry-focused practices.

  • Bilingual skills are a plus.
