JOB TITLE: Centralized Referral Clinical Coordinator (Licensed)

JOB SUMMARY: The Centralized Referral Coordinator serves as the first point of contact for electronic patient referrals. They will use their knowledge and experience within a skilled nursing organization to process incoming referrals timely for clinical and financial appropriateness for admission into our skilled nursing communities. 

ESSENTIAL FUNCTIONS: 

•  Process all incoming electronic placement request referrals for financial and clinical acceptance in accordance with Caraday’s expectations. 
•  Ensure complete and effective communication regarding placement requests to referring entity and Caraday’s approved community. 
•  Data entry as it relates to referral processing. 

Requirements

QUALIFICATIONS: 

Required 

•  Current LVN/RN license with Skilled Nursing Facility experience 
•  General knowledge of financial requirements for admissions to skilled nursing 
•  Experience with Point Click Care 
•  Working knowledge of Microsoft Office suite, Microsoft Teams, SharePoint 

Preferred 

•  MDS or ADON experience 

PHYSICAL REQUIREMENTS: The ability to safely perform movements such as pushing, pulling, lifting, bending, kneeling, and reaching up to 50 pounds with or without reasonable accommodations 

WORK SCHEDULE: Full Time Exempt Monday to Friday 

WORK ENVIRONMENT: General Office
