Referral Coordinator helps to complete referral orders, as well as assist in the care coordination needs of our patient population with chronic health problems. A referral coordinator is an important member of the health care team and is expected to demonstrate a high level of engagement with their co-workers, providers, and patients/families. Referral Coordinators guide our patients through a process that may be difficult for the patient to navigate.   Additionally, some critical thinking skills are necessary in order to follow standing orders and algorithms for screening, prevention, and care planning.

Responsibilities (included, but not limited to):

• Oversees the data entry and completion of referral forms/notifications.
  


• Coordinates the process of referring patients to appropriate clinics.


• Communicates with patients as to any pre-test/referral instructions and to ensure follow-through with referrals.


• Updates records to assure all parties have accurate information.
  


• Verifies insurance benefits, obtains initial authorizations, and inputs all pertinent insurance information into the EMR.


• Ensures practitioners are following standardized guidelines for referrals as recommended by evidenced based medicine.


• The Referral Coordinator employs a process that tracks referrals, communicates with the providers referred to, ensures that an appointment is made, and recovers relevant patient information for the patient medical record maintained at Fordland Clinic. 
  


• Patient referrals are to be monitored by the referral coordinator, and the order status updated once an appointment has been set by the receiving facility/provider. 
  


• Act as liaison between primary care provider and alternative care 


• Maintain knowledge of insurance requirements. 


• Build relationships with patients and resolve conflict.


• Communicate and educate families about the treatment plan being delivered to the patient

Education and Experience Requirements

• High school diploma


• Experience in customer service


• Experience in health care office preferred

Work hours: Full-time, hourly position. Clinic hours are 8:00 am to 6:00 pm Monday through Friday, and 8:00 am to 4:00 pm Saturday. Current need for this role is Monday - Friday 9 am - 5pm.
