Job Description - Referral Coordinator

Location: Lakeport - Lake County, CA
Organization: Mendocino Community Health Clinic (MCHC)
Employment Type: Full-Time

Are you passionate about connecting people to the care they need? Mendocino Community Health Clinic (MCHC) is looking for a dedicated and detail-oriented Referral Coordinator to join our team! In this role, you'll play a key part in coordinating patient referrals to specialty care, helping ensure every patient receives timely, high-quality healthcare.

What You’ll Do:

  • Coordinate referrals and schedule specialty appointments for patients.
  • Communicate with external medical offices to ensure seamless referral processes.
  • Confirm financial eligibility and obtain insurance authorizations as needed.
  • Provide patients with appointment details, including maps and preparation instructions.
  • Serve as a liaison between providers, patients, and outside healthcare organizations.
  • Maintain accurate documentation and ensure medical information is prepared and shared appropriately.
  • Assist providers and staff by troubleshooting referral-related issues.
  • Travel to other MCHC sites as needed.

Requirements

Minimum Qualifications:

  • High School Diploma or GED.
  • 2+ years of relevant experience, or a combination of education and training.
  • At least 1 year of experience in customer service or client relations.

Preferred Qualifications:

  • 1+ year of experience working in a healthcare environment.
