Job Description - Referral Coordinator

Job Title: Referral Coordinator
Location: Fortuna Family Medicine, Inc.
Job Type: Full-Time
Reports To: Front Office Manager



Position Summary:


Fortuna Family Medicine, Inc. is seeking a detail-oriented and proactive Referral Coordinator to join our primary care team. This role is essential in coordinating referrals from our office to specialty providers, ensuring follow-up, tracking referral statuses, and maintaining compliance with healthcare regulations. The ideal candidate will also be the point of contact for patient inquiries related to referrals and orders.



Key Responsibilities:




  • Coordinate outgoing referrals from primary care to specialty providers and ancillary services.


  • Ensure all referrals are authorized, scheduled, and tracked through completion.


  • Follow up with patients and specialty offices to verify appointment attendance and obtain visit reports.


  • Maintain accurate and up-to-date records of all referrals, diagnostic orders, and specialist reports in the EHR.


  • Work closely with providers and clinical staff to ensure timely communication of referral outcomes.


  • Handle incoming calls related to referrals, imaging, labs, and other patient inquiries.


  • Ensure compliance with HIPAA, clinic policies, and health plan requirements regarding referrals and documentation.


  • Communicate with insurance companies as needed for referral or authorization processes.


  • Assist with reporting and audits related to referral metrics and quality initiatives.


  • Support patient education regarding their referrals and what to expect from specialist visits.



Qualifications:




  • High School Diploma or equivalent required; some college or medical office training preferred.


  • 1–2 years of experience in a medical office, in referrals or care coordination.


  • Strong understanding of healthcare referral processes, insurance requirements, and EHR systems (e.g., EPIC, eClinicalWorks, etc.).


  • Excellent communication, organizational, and follow-up skills.


  • Ability to multitask and prioritize in a fast-paced environment.


  • Detail-oriented and committed to maintaining patient confidentiality and regulatory compliance.



Preferred Skills:



  • Familiarity with local specialists and healthcare networks


  • Knowledge of medical terminology and insurance authorization workflows
