What You’ll Do as a Care Logistics Referral Coordinator You’ll play a key role in connecting patients with the care they need, supporting their journey to better health. Responsibilities include: Managing inbound and outbound referrals for specialty providers. Scheduling specialty services and maintaining regular communication with providers. Ensuring referral tasks are completed within 72 business hours. Documenting referral details in our EMR system. Answering patient inquiries with kindness and professionalism. Supporting team members as needed. What We’re Looking For Proficiency with Microsoft Word, Excel, and EMR systems (preferred). Bilingual (Spanish/English) candidates are highly desired.
