Position/Title: Referral Coordinator/Medical Assistant

MAJOR FUNCTION: The Referral Coordinator provides medical support services to assist patient with managing health.
QUALIFICATIONS:
ESSENTIAL RESPONSIBILITES:
· Maintains a referral log
· Assist with call backs, to patient’s insurances and outside providers when referring to all referrals
· Maintain EHR referral inbox
· Initiates referral in timely manner
· Refer patient to providers that participate with their insurance plan
· Make referral arrangement if needed (scheduling appointments)
· Assist with closing the referral loop
· Generate reports from referral module
· Actively participate in all require meetings
· Responding to email from referral module pertaining to referral submitted
OTHER JOB DUTIES:
· Ability to sit for long periods of time
· Other duties as assigned by department head
