Position Summary
We are seeking a compassionate and organized Referral Navigator to join our care team. This individual plays a vital role in coordinating specialty medical services and diagnostic testing for our patients, ensuring seamless access to the care they need. The Referral Navigator serves as a liaison between patients, providers, and external specialists, and will support our mission by offering personalized, accessible service grounded in love and respect.
Key Responsibilities
Meet with patients to schedule specialty services or tests as ordered by medical providers
- Coordinate appointment dates, times, and financial arrangements with specialty providers
- Share medical records, referral requests, and orders to specialists in a timely, HIPAA-compliant manner
- Educate patients on the basic requirements for procedures such as CT scans, MRIs, and X-rays
- Track referrals and appointments through the Electronic Medical Record (EMR) system
- Collaborate with Case Managers to assist patients with transportation, counseling, and other supportive services
- Support Patient-Centered Medical Home (PCMH) efforts by contributing to process development and patient education
- Follow up with outside providers to ensure patient compliance and collect test results
- Participate in organizational training (PHCC, OSHA, Emergency Preparedness, etc.)
- Demonstrate initiative, professionalism, and teamwork in all interactions
Qualifications
Knowledge of basic medical terminology and office procedures
- Proficiency with Microsoft Word and Excel
- Strong organizational and communication skills
- Experience with referrals and medical office systems (preferred)
- Ability to maintain strict patient confidentiality
