We're looking for a clinical leader who thrives on building strong teams and delivering exceptional patient care. As Regional Director of Patient Care Services, you'll oversee clinical operations across multiple home health service lines, guiding a team of Clinical Supervisors and ensuring patients receive safe, high-quality care in their homes.
This is a hands-on leadership role—you'll shape how care is delivered, mentor the next generation of clinical managers, and drive the outcomes that matter most to patients and families.
What you'll do:
Lead & Develop Your Team
- Directly supervise 4–5 Clinical Supervisors across different service modalities
- Coach, mentor, and grow leadership capabilities within your team
- Conduct regular one-on-ones, performance reviews, and support accountability across branches
Drive Clinical Excellence
- Ensure care plans align with physician orders, agency policies, and patient needs
- Step in on complex or escalated cases to guide clinical decision-making
- Implement best practices and standardized care delivery models
Own Quality & Compliance
- Monitor key metrics: rehospitalization rates, patient satisfaction, timely start of care, OASIS accuracy, and star ratings
- Keep the region survey-ready and lead corrective action when needed
- Ensure compliance with federal/state regulations, Conditions of Participation, and infection prevention standards
Optimize Operations
- Balance staffing, scheduling, and caseloads to match census and acuity
- Partner with leadership on productivity, resource allocation, and workflow efficiency
- Support financial performance goals while protecting care quality
Collaborate Across the Organization
- Work closely with intake, quality, compliance, HR, and business development teams
- Coordinate with physicians, hospitals, and community partners
- Facilitate interdisciplinary care and communication
In Order to be a Success:
- Active California RN or PT License
- 3+ years of management experience in home health or a related healthcare setting (within the last 5 years), including at least 1 year in a supervisory or administrative role
- Strong critical thinking, organization, and interpersonal skills
- Valid California driver's license, auto insurance, and clean driving record
Preferred
- Deep clinical knowledge specific to home health
- Proven leadership in multi-site or multi-disciplinary environments
