SUMMARY
Granite United Way, in collaboration with NH Care Connections, New Hampshire Department of Health and Human Services, and Unite Us, is seeking a Regional Engagement Coordinator passionate about public health and care coordination. This role supports implementation of Unite Us’ Closed Loop Referral platform, connecting residents to services that address social determinants of health while improving and tracking outcomes.
We are looking for a flexible, dependable professional who thrives in collaborative settings while demonstrating strong project management, outreach and presentation skills. The ideal candidate will build trusting relationships with community-based organizations, clinical providers, and government partners, communicates clearly and confidently, and successfully cultivates local buy-in to strengthen networks and improve care for New Hampshire residents.
RESPONSIBILITIES
Network Development & Partner Onboarding
- In partnership with the Unite Us team, identify, cultivate, and onboard strategic priority partners to strengthen and expand the NH Care Connections network.
- Identify key organizations and influencers across core service domains to support equitable network participation and coverage.
- Serve as a partner advocate to promote adoption and meaningful use of the Unite Us platform within network organizations and their community partners.
Engagement, Convening & Community of Practice
- Plan, promote, and facilitate provider interest meetings and ongoing engagement opportunities to maintain momentum and responsiveness to community needs.
- In partnership with the Unite Us team, convene and facilitate an ongoing provider community of practice to strengthen engagement and coordinated referral pathways across the referral network.
- Collaborate with the Unite Us team to plan, host, promote, and/or present quarterly virtual Community Conversations to grow network participation and increase utilization of network resources, including the CONNECT Navigator partnership.
Platform Training & Demonstration
- Complete required training to develop strong proficiency in the Unite Us platform and referral workflows.
- Conduct live demonstrations and presentations of the Unite Us platform for community partners to support onboarding, adoption, and effective use.
- Provide high-level guidance and support to partners navigating platform functionality, in coordination with the Unite Us team.
Data-Informed Growth & Strategy
- Work closely with the Unite Us team to analyze network data and utilization trends, and provide recommendations to strengthen adoption, close service gaps, and increase active participation.
- Maintain awareness of regional needs, emerging priorities, and stakeholder feedback to inform network growth strategies.
Cross-Sector Collaboration & Alignment
- Build and maintain collaborative relationships across diverse sectors including health care, behavioral health, social services, education, government, and business.
- Leverage existing communication channels and develop new outreach strategies to advance regional engagement and network growth goals.
- Align network engagement efforts with Regional Public Health Network priorities and broader community health improvement strategies.
- Participate in relevant education, training, and technical assistance opportunities to strengthen coordination and system alignment.
EDUCATION AND EXPERIENCE
- A bachelor's degree or equivalent experience in human services, healthcare and/or business administration.
- Experience quickly building social capital and coordinating projects.
- Experience engaging diverse audiences and generating buy-in across multiple stakeholder groups.
Preferred
- Experience aligning initiatives with broader regional and public health strategies
- Familiarity analyzing data, identify trends, gaps, and opportunities, and utilizing these metrics to guide strategy
- Knowledge of cross-sector referral networks and coordinated care models
- Understanding of training and adult learning principles for effective platform demonstrations and onboarding

ADDITIONAL ELIGIBILITY REQUIREMENTS
- Valid driver’s license, reliable transportation, and proof of insurance
SCHEDULE & TRAVEL REQUIREMENTS
- GUW’s core business hours are between 8:30am and 4:30pm Monday-Friday. However, the Regional Engagement Coordinator role must be available to work a flexible schedule, which may include night and weekends.
- Willingness to travel within assigned regions across Granite United Way’s service area in New Hampshire, required.
