Summary:
The Certified and enrolled Dietitian provides evidence-based nutrition assessment, counseling, education, and medical nutrition therapy (MNT) to patients across the lifespan in a multidisciplinary healthcare setting, and school-based health centers. The dietitian works collaboratively with physicians, advanced practice providers, nurses, behavioral health clinicians, care coordinators and community health workers to improve health outcomes, manage chronic disease, and promote disease prevention for a diverse patient population.
Essential Duties & Responsibilities:
*All agency staff are required to attend all mandatory department/agency meetings and trainings*
Schedule:
Monday - Friday, 8:30 AM - 5:00 PM
Education &/or Experience:
Minimum of 3 years of experience. Master’s degree is preferred in Nutrition, Dietetics, or related field. ?Certified, and enrolled dietitian-nutritionist under the Connecticut Medical Assistance Program (CMAP) Licensed Dietitian/ Nutritionist in Connecticut. Current CPR certification. Experience in primary care, community health, FQHC, or School-based health center. Bilingual, Spanish speaking, preferred.
