Key Responsibilities

  • Complete initial, periodic, and change-in-condition nutrition assessments, evaluating dietary intake, weight trends, hydration, medical history, and access to food.

  • Develop, implement, and monitor individualized nutrition care plans based on participants’ clinical needs, cultural preferences, and personal goals.

  • Provide nutrition counseling and practical education to participants and caregivers on chronic disease management, therapeutic diets, and healthy eating.

  • Monitor participants at nutritional risk, including those experiencing unintended weight changes, poor intake, dehydration, or difficulty meeting dietary needs.

  • Participate in IDT meetings, communicate nutrition concerns, and recommend adjustments to participant care plans.

  • Collaborate with food service staff to support appropriate meals, therapeutic diets, dietary modifications, and nutritional adequacy.

  • Maintain accurate, timely documentation of assessments, interventions, participant progress, and follow-up care.

  • Support quality improvement activities and adherence to organizational policies and applicable PACE requirements.

Required Qualifications

  • Current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential through the Commission on Dietetic Registration.

  • Completion of education and supervised practice requirements for RD/RDN credentialing.

  • Strong nutrition assessment, counseling, and care planning skills.

  • Ability to interpret clinical information and translate recommendations into practical, participant-centered interventions.

  • Strong communication, organization, and documentation skills.

  • Ability to work collaboratively with an interdisciplinary healthcare team.

Preferred Qualifications

  • Experience in PACE, geriatric care, community health, long-term care, or a related setting.

  • Experience supporting individuals with multiple chronic conditions and diverse nutritional needs.

  • Bilingual English/Spanish skills.

  • Experience with electronic health records and interdisciplinary care planning.

Work Environment and Schedule

This position supports an onsite, community-based PACE program. Responsibilities may include participant visits in the center and other care settings, depending on program needs.
