Summary
Provide general nursing care to patients during and between clinic visits by performing the following duties.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
Coordination of care of patients between clinic visits.
Triage patients who phone in or walk in with problems.
Work in clinic on dates assigned.
Chaperone male providers during intimate examinations of female patients.
Maintain awareness of comfort and safety needs of patient.
Authorize refills of medications according to clinic protocols.
Document in EHR database.
Facilitate Patient Assistance Program applications for medications as needed.
Participate in Continuous Quality Management program.
Communicate with Case Managers to ensure all patient needs are met, including with regards to medications, plan of care, providers’ orders.
This position may require the employee to transport patient information in their personal or company vehicle. This information must be secured during transport.
Supervisory Responsibilities
This job has no supervisory responsibilities.
Need to Know Access
The RN Care Manager will have access to the shared network, the CAREWare database for clinical data review and the EHR portal for data entry and medication ordering. She/he will also have access to the patient/client insurance screening portal.

Education and/or Experience
Associate's degree or equivalent from two-year college or technical school;1 (one) year related experience.
Language Skills
Ability to read, analyze and interpret medical documents. Excellent written and verbal communication skills. Excellent documentation.
Certificates, Licenses, Registrations
Current active, unencumbered Florida RN license.
