Job Description - Registered Nurse

General Statement of Duties

The Registered Nurse provides nursing care to patients utilizing the nursing process of assessment, planning, intervention and evaluations. The Registered Nurse carries out tasks as directed by the medical provider. He/she will provide case management services as a liaison between the patient and provider to resolve care concerns as they arise and to provide health education to the patient as indicated. The Registered Nurse serves as a resource to other clinical staff.

Responsibilities and Essential Functions

  • Reviews      patient history, medication list, diagnostics, and outside records in      order to implement an appropriate plan of care for the patient as defined      by the provider.
  • Identifies individual care needs based on assessment outcomes as requested by provider.
  • Explains procedures and treatments to patients and provides education on various health-related topics.
  • Communicates with providers, patients and family members regarding care plans, goals, and expected outcomes.
  • Oversees the Point of Care schedule and coordinates all POC testing and visits under the supervision of the provider.
  • Coordinates Chronic Care Management (CCM) services for patients enrolled in CCM. Identifies eligible patients for CCM and coordinates the development of individualized care plans to manage long-term conditions. 
  • Manages anticoagulation clinic under the supervision of the provider.
  • Manages hospital tracking tool and coordinates follow-up appointments with appropriate provider or service.
  • Assists providers with Obstetric services and coordination for patients.
  • Documents all activity in medical record as appropriate and in accordance with medical records standards.
  • Responds to patient telephone calls as needed.
  • Acts as advocate on behalf of patients. 
  • Responds to urgent, emergent, or life-threatening situations when they arise.
  • Assists department with duties and coverage as needed.
  • Maintains strict patient confidentiality.
  • Performs other duties as assigned.

Requirements

  Minimum Qualifications and Other Essential Functions

  •  Establish and maintain effective and harmonious working relationships with staff, patients, vendors, and the public
  • Maintain excellent oral and written communication skills and an ability to practice effective professional communication
  • Thrive and promote group cohesion as a team member in a rapidly changing environment
  • Follow detailed and written oral instructions
  • Multi-task and adjust priorities in a fast-paced environment, while maintaining focus and managing disruptions and/or unexpected needs
  • Accept feedback from a variety of sources and constructively manage any conflicts 
  • Maintain excellent organization skills
  • Execute and track detail-oriented projects and deadlines
  • Demonstrate professionalism
  • Demonstrate good judgement while working independently or as part of a team 
  • Maintain punctual attendance
  • Maintain general computer and keyboarding skills
  • Must be BLS certified 
  • Must hold a current Oregon State Nursing License 

Preferred Qualifications

  • Bachelor/Associate degree 
  • Two years of acute care nursing
  • Experience      with Chronic Disease Management in a clinical setting
  • Experience in Primary Care Triage
  • Intermediate or advanced knowledge of Microsoft Office Products: Excel, Outlook, Word, and Power Point
  • Knowledge of Federally Qualified Health Centers

Physical Demands Required to Fulfill Essential Functions of this Position

Employee must be able to: sit or stand for long periods of time; focus on tasks while in an active office environment where conversation and noise is prevalent; operate a keyboard, write, speak, and hear; read small print both on paper and on a computer screen for long periods of time and, occasionally lift up to 20 pounds. 

Additional Requirements

· Submit to and pass a drug test

· Successfully complete a criminal background check

· Maintain HIPAA compliance and follow confidentiality policies to protect organizational information

· Foster ethical behavior, cultural sensitivity, and an inclusive environment in accordance with our Standards of Conduct and Respectful Workplace Policies

· Work beyond normal working hours, including weekends, if applicable and when required

Working Conditions

There may be exposure to airborne and blood-borne pathogens, and hazardous materials. This position may include working remotely.
