Job Summary

**This role requires driving to patient's home to render all services.**
**All services will be rendered within the DFW area**

We are seeking a Registered Nurse to join our team. In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. You take pride in providing nursing that is in accordance with the patient's plan of care, to include comprehensive health and psychosocial evaluation, monitoring patient's condition while providing teachings, trainings and direct nursing care.

Responsibilities

Coordinate total patient care by conducting comprehensive health and psychosocial evaluation.

Perform admission, transfer, re-certification, resumption of care, and discharge OASIS for home care patient.

Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans

Consult with the attending physician, concerning alternations of Patient Care Plans, check with the appropriate supervisor, make changes as appropriate.

Submit clinical notes, progress notes and other clinical record forms outlining the services rendered, in timely fashion.

Document any changes in patient conditions and report on changes to appropriate supervisors

Maintain accurate records of patient care and discuss observations with the supervisor

Qualifications

Currently licensed Registered Nurse (RN) in state of employment

Previous experience as a Home Health Care Registered Nurse

Excellent interpersonal and client care skills

Experience in OASIS Comprehensive assessments

Valid driver’s license

First aid/CPR certification required

Compensation: $35.00 - $45.00 per hour
