Description
· Assist healthcare professionals in providing patient care to individuals diagnosed with diabetes, primarily via phone.
· Perform routine clinical tasks, including collecting patient history, assessing patient needs, and accurately documenting information in electronic health records.
· Provide general education to patients about diabetes management, including medication administration, insulin usage, blood sugar monitoring, recognizing signs of hypo/hyperglycemia, healthy eating habits, and lifestyle modifications.
· Offer clinical support and encouragement to patients, helping them navigate the challenges of living with type 2 diabetes.
· Schedule follow-up appointments, coordinate care plans, and ensure efficient communication between patients, healthcare providers, and the care team.
· Stay current with the latest advancements in diabetes management and treatment to provide evidence-based assistance to patients and support the healthcare team.
· Adhere to all HIPAA regulations and maintain strict confidentiality of patient information.
· Stay up to date with the latest advancements in diabetes management and treatment to provide informed assistance to patients and the healthcare team.
· Adhere to all privacy regulations and always maintain confidentiality of patient information.
· Comfortable spending much of the day assisting patients via phone.
· Other duties as assigned.
Requirements
What You'll Bring
· Active Registered Nurse (RN) license in [state or multi-state compact]
Associate's or Bachelor’s degree in Nursing required.
· Previous experience working with patients with diabetes and Continuous Glucose Monitors (CGM’s) preferred
· CDCES certification is a plus.
· Experience in a remote patient monitoring or telehealth setting is a plus.
· Strong clinical assessment skills with the ability to triage and escalate care when needed.
· Comfortable spending majority of day assisting patients via phone.
· Strong interpersonal and communication skills to effectively interact with patients, healthcare professionals, and other team members.
· Compassionate and empathetic nature, with a genuine desire to help patients manage their diabetes effectively.
· Excellent organizational skills and attention to detail.
· Ability to work efficiently in a fast-paced environment.
· High school diploma or equivalent.
