Key Responsibilities

• Deliver skilled nursing care to patients in their homes


• Complete and submit accurate OASIS assessments and documentation


• Develop and implement individualized care plans


• Educate patients and caregivers on diagnoses, medications, and self-care


• Collaborate with the interdisciplinary care team for the best patient outcomes


• Document visits and care using Kinnser/WellSky EMR

Qualifications

• Active Registered Nurse (RN) license in the state of Arizona


• Minimum of 1 year of home health experience, including OASIS documentation (preferred)


• Strong knowledge of Medicare guidelines and home health regulations


• Reliable transportation and a valid driver’s license


• Excellent clinical, communication, and organizational skills


• Must be willing to see patients in Boyton Beach!
