Registered Nurse

Role: Registered Nurse – Health Risk Assessment Specialist

Location/Type: Wichita, KS • Field-based (in-home visits)

Pay: $90 per completed visit + mileage reimbursement

Schedule: Full-time • Monday–Friday • 8:00–5:00

  Conduct in-home health risk assessments with Medicaid members

  
  


• 
  

  Review medical history, medications, hospitalizations, and specialists

  
  


• 
  

  Screen for mental health/substance use (PHQ-2, others)

  
  


• 
  

  Create care plans with SMART goals across domains

  
  


• 
  

  Assess social needs: housing, food, transport, work

  
  


• 
  

  Document advance directives and emergency plans

  
  


• 
  

  Coordinate with care teams and community resources

  
  

• 
  

  Active Kansas RN license (unrestricted)

  
  


• 
  

  3+ years clinical nursing experience

  
  


• 
  

  2+ years Medicaid/managed care experience

  
  


• 
  

  Comfort with adult and pediatric assessments

  
  


• 
  

  Strong assessment and documentation skills

  
  


• 
  

  Reliable transportation and ability to travel locally for home visits

  
  

• 
  

  Behavioral health or FQHC background

  
  


• 
  

  Case management or care coordination experience

  
  


• 
  

  Bilingual (Spanish preferred)

  
  

• 
  

  $90 per completed visit (contractor, 1099)
