Responsibilities:
Oversee the patient's care plan implementation and assessment
Collaborate with the interdisciplinary team for comprehensive patient support
Develop and execute tailored care plans
Communicate patient progress and coordinate care adjustments
Engage in team meetings and supervise staff

Requirements:
Active RN license in Pennsylvania
CPR/FA certification
Valid driver's license and reliable transportation
1 year of RN experience in hospice or similar setting

Sign-On/Retention Bonus Options:
1 year - $5,000
2 years - $10,000
3 years - $15,000

Pay:

$65,000 - $85,000 per year
