S/he interfaces with community providers, insurance companies and agencies and providers along the care continuum.# Direct the work of LPN Care Coordinators.# Participate in Patient Centered Medical Home activities. Minimum Qualifications:# Current RN Licensure in New York; Associates Degree in Nursing, Bachelor#s Degree in Nursing (BSN) preferred,#Certification in Case Management (CCM, ACM) preferred,##Knowledge of provider community and community resources in the Syracuse NY/CNY areas,#Self-disciplined, energetic, passionate, and innovative,#Confidence to communicate and outreach to other community health care organizations and personnel,#Able to maintain confidentiality with all aspects of information in accordance with Practice, State and Federal regulations,#Demonstrates sound judgement, decision making, and problem solving skills,#Highly organized and well developed oral and written skills,#Comprehensive knowledge of insurance company principles and outpatient delivery systems Preferred Qualification:## Minimum 5 years clinical experience This is a remote position, but the candidate hired must live in Central New York. Pay Range: $58,000.00 - $72,000.00#DOE#
