Our client is the only not-for-profit, community-based hospice in the Roanoke and New River Valleys and SML. For 12 years straight, we have won gold in the Our Health Senior Living Awards category for hospice.  We are seeking an RN Care Manager for our Roanoke location to bring comfort, compassion, and quality care to patients and families when they need it most.

As an RN Case Manager, your ability to manage patient care and provide services requires substantial, specialized bedside nursing skills for symptom management, crisis intervention, and family education. In addition to excellent written/verbal communication, problem-solving, and decision-making abilities, the candidate should demonstrate experience with developing and maintaining rapport with patients and families, and the ability to work well with an interdisciplinary team.

WORKING CONDITIONS

• Primarily works in the patient’s residence. 


•  Travel to long-term care facilities, hospitals, and area agencies and businesses will be necessary. 


• Travel within the entire Hospice service area will be necessary. 


•  Overnight travel outside of the area may be necessary for continuing education.


• The employee is at direct risk for exposure to infections and blood-borne pathogens.

·       Graduate from an accredited school of nursing

·       Currently licensed to practice nursing by the Virginia State Board of Nursing,

·       Two years’ experience in acute care, home health, hospice, med/surg or geriatric nursing

·       Current Virginia driver’s license and driving record that meets the standards set forth in current policies.

Salary

•     Salary: $34 - $36/hour


•     Scrub Color: No specific color --- Dress Code business casual
