Job Overview

This is an opportunity for a dedicated Registered Nurse Case Manager to join our client and make a difference in the lives of patients in hospice care. You will be part of a supportive team, providing essential care and coordination for patients and their families. Key Responsibilities â Manage patient care in hospice settings â Coordinate with healthcare teams â Provide leadership and support to nursing staff Schedule and Shift Details Schedule: Monday â Friday 8:00 am â 6:00 pm, 1 weekend per month, and 2 holidays per year Travel â Local travel required covering multiple counties.

Requirements

Qualifications

High School/GED

Hospice (1+ years)

Management (1+ years)

1+ years of experience in ANY of the following:
- Home Health Care
- Epic Home Health
- Home Health

Valid Registered Nurse (RN) in the state of Pennsylvania (PA)

ALL of the following valid licenses/certifications:
- American Heart Association Basic Life Support (AHA BLS)
- American Heart Association Advanced Cardiac Life Support (AHA ACLS)

Have you applied to or worked for Residential Healthcare Group anytime in the last 6 months? (no)

Are you okay with the required travel for this position? (yes)
