Key Responsibilities:

• Perform skilled nursing visits and in-home assessments
  
• Deliver wound care, medication administration, and health monitoring
  
• Educate patients and caregivers on disease management and home safety
  
• Collaborate with interdisciplinary providers and community partners
  
• Maintain up-to-date records through our electronic documentation system
  

What You Bring:

• Valid RN license in the State of Minnesota
  
• At least 1 year of clinical nursing experience (home care preferred)
  
• Reliable vehicle and driver’s license
  
• Strong independent decision-making skills
  
• Passion for personalized care and meaningful patient relationships
