Your Day-to-Day

• Perform comprehensive nursing assessments addressing physical, emotional, psychosocial, and spiritual needs
• Collaborate with interdisciplinary healthcare teams to develop, implement, and evaluate individualized care plans
• Provide skilled nursing services including:
  • Wound care
  • Medication management
  • IV therapy
  • Symptom and pain management
  • Patient and family education
• Deliver compassionate emotional support and education tailored to patient and caregiver needs
• Supervise LPNs/LVNs and home health aides while coordinating care activities
• Respond to urgent patient needs and participate in admissions and on-call responsibilities as required
• Monitor patient progress and adjust care plans based on ongoing assessments
• Maintain accurate, compliant clinical documentation and records
• Ensure the highest standards of quality, safety, and compassionate care

What We're Looking For

• Graduate of an approved school of professional nursing
• Current Registered Nurse (RN) license in the state of agency operation
• Minimum of one (1) year of RN experience
• Current CPR certification (or ability to maintain certification while employed)
• Valid driver's license
• Reliable, insured automobile in accordance with state and organizational requirements
• Ability to travel to patient homes and business locations as needed
• Strong clinical assessment, communication, and care coordination skills
