Job Description:
Responsible for collecting and analyzing patient data to formulate individualized care plans. Determines progress toward outcomes and coordinates care delivery. Promotes a safe practice environment and advocates mutual respect with patients, families, and healthcare providers.
• Conducts in-home assessments and develops, implements, and evaluates individualized care plans in partnership with patients, representatives, and caregivers.
• Communicates significant clinical changes or findings to physicians and other healthcare practitioners involved in the plan of care.
• Educates patients and families on disease management, medication administration, and preventative care.
• Monitors vital signs, observes for changes in health condition, and provides wound care as needed.
• Collaborates with healthcare professionals to ensure holistic and coordinated patient care.
• Maintains accurate and timely medical records and documentation.
• Supervises and instructs home health aides and student nurses as directed.
• Takes on-call responsibilities during evenings, nights, holidays, and weekends on a rotating basis.
• Performs other duties as assigned.
Associate’s degree in nursing
Bachelor’s degree in nursing (BSN) is preferred
0-2 years of experience in nursing, or a relevant area
An equivalent combination of education and experience may be considered.
All experience must be paid and in the same related field.
Part-time and PRN experience will be prorated based on hours worked per week.
Volunteer work and internships for academic credit are not counted.
Current and active Registered Nurse (RN) license, Basic Life Support (BLS)
