About the Role:

 As an Infusion and Home Care SWAT RN with us, you will play a crucial role in delivering high-quality care and responding to a variety of patient needs across different units. This position offers a Monday - Friday schedule with rotating weekends, allowing for a balanced work-life experience. You will be required to provide coverage across all valley locations, ensuring comprehensive patient care throughout our network.

A minimum of 2 years of home care RN experience is required for this position. Work at your own pace, providing quality care and education in the comfort of patients' homes. Must be willing to travel within an assigned area in the valley, but don't worry, you'll be compensated for mileage. Mileage gets reimbursed for every mile in between the first and last 20 miles of your shift. But it's not just about the compensation it's about the meaningful work we do together. Pay:

$80,000 to $100,000 annually

Responsibilities:

• Provide direct patient care and support to various departments
• Respond swiftly to patient care needs and emergencies
• Collaborate with healthcare team members to ensure optimal patient outcomes
• Conduct thorough assessments and administer treatments
• Maintain accurate and detailed patient records
• Educate patients and their families on health management and treatment plans
• Travel to different valley locations as needed to provide coverage

Available Shifts:

• 3 PM - 11 PM, Monday - Friday, rotating weekends, with agency on-call requirements

 Preferred Qualifications:

• Proven excellence in customer service with a strong focus on patient satisfaction
• Positive, proactive attitude and exceptional organizational skills
• Deep passion for the nursing profession and dedication to quality care
• Proficiency in Medicare OASIS documentation, infusion therapy, and wound care is a plus
• Minimum of 2 years of experience in home care nursing required
• Valid Arizona Level One Fingerprint Clearance Card
• Active Driver's License
• Current CPR/BLS certification
• Proof of car insurance
• Valid TB test results
• First Aid certification
• Active RN license in Arizona
