Job Description - Registered Nurse - Interim Housing Program (ESP) - Generous Compensation

Position Purpose: The Interim Housing Registered Nurse will participate as a care team member of the Enhanced Services Program, which provides intensive case management services, behavioral healthcare and offers a satellite medical clinic, on-site. This is funded by DHS, in the Housing for Health program.

Program Population:

The ESP Program- is an Interim Housing program that aids homeless individuals in their recovery journey, helping them improve their health conditions and enhance their social and activities of daily living (ADL) skills, all with the ultimate aim of securing stable housing opportunities. Some of these individuals may also grapple with mental health and substance abuse issues, which can occasionally manifest in behaviors such as yelling, using strong language, and displaying anger outbursts. Patients must be homeless, have an acute medical condition, and medically and physically stable. This respite program provides medical care, case management, personal care and access to long-term housing resources and placement, Wesley also provides these patients with transportation to and from medical visits and to specialists and other social services agencies in the County.

Principal Responsibilities:

  1. Conducts triage for all walk-in patients; including vital signs and complete assessments.
  2. Participates in all case conferences regarding ESP clients, supporting care teams in treatment plans.
  3. Completes TB screenings/readings and documentation.
  4. Administers oral, IM, SC, and SL medications as needed.
  5. Assists the Medical Providers with procedures as necessary
  6. I.V. placement and monitoring; including the infusion of I.V. fluids as ordered by the Provider.
  7. Completes corrective action plans, if any, post internal/external audits.
  8. Monitors the medication management process to assist clients/patients in medication self-administration.
  9. Provides guidance and support to ESP staff regarding medical care, as needed.
  10. Takes patient complaints and follows appropriate procedures in resolving any complaint.
  11. Provides patient education and teaching. Schedules patient hospital or doctors (specialist) appointment, if necessary.
  12. Assigns daily nursing duties. Identifies and corrects patient flow deficiencies.
  13. Organizes and maintains dispensary medication inventory; to include removing expired medications and packaging them for proper disposal.
  14. Preparation and dispensing of all medications as ordered by the Provider; to include the completion of dispensed medication labels.
  15. Places orders for medical supplies as needed.
  16. Collaborates with the Program Director and assists with the continuing improvement of systematic functions.
  17. Develops and monitors the communication network between the multi-disciplinary ESP staff and external professionals to improve continuity of care and to avoid conflicting information conveyed to clients/patients regarding their treatment plan.
  18. Ensures all PHI information is maintained in accordance with HIPAA regulations.
  19. Supports program leadership in delivering all required services and meets outcome measures established by the Department of Health Services.
  20. Ensures quality measures, regulatory standards, and customer service standards are met.
  21. As needed, assists Program Director with reports on operational issues, organizational opportunities, and/or programmatic goals.
  22. Monitors and maintains adherence to the Nursing Training Manual and Agency Policies and Procedures.
  23. Performs other duties as required by the program and/or clinic.

Requirements:

  1. Licensed by the State of California to practice as a Registered Nurse.
  2. CPR Certificate by America Heart Association
  3. At least one year experience in an ambulatory care setting.
  4. Experience working with patients and staff from diverse socio-economic, ethnic, and cultural backgrounds.
  5. Experience working with chronically homeless individuals
  6. Strong understanding of case management principles and practices.
  7. Ability to work collaboratively as a member of a multidisciplinary care team.
  8. Excellent communication and interpersonal skills.
  9. Proficiency in electronic health record (EHR) systems.
  10. Knowledge of and adherence to HIPAA regulations and patient confidentiality.
  11. Commitment to providing compassionate and culturally sensitive care to underserved populations.
  12. Familiarity with Housing for Health programs or similar initiatives is a plus.
  13. Bilingual (English/Spanish) preferred.
