Position Summary:
Responsible for coordinating medication/health needs of a caseload of 10+ clients. This is an in-person position working 5 days per week.
Salary Range: Min $90,000 | Mid $100,000 | Max $110,000
Essential Functions and Responsibilities:
Focusing on issues related to psychiatric medications and physical health, provide direct clinical services to clients on an individual basis in the office, in their homes, and in community settings.
Provide ongoing assessment of clients’ physical health symptoms and clients’ response to treatment. Make appropriate changes to the treatment plan to ensure immediate interventions are provided in response to changes in medication/health.
Provide management and administration of medication, in conjunction with team psychiatrist, including pharmacy coordination and insurance navigation Independently problem solve when issues arise.
Provide medication/health education to clients and families.
Conduct holistic and comprehensive medical assessment along with other team members
Educate and support clients’ families and advocate for clients’ rights and preferences
For an assigned group of clients, assume primary responsibility for case management and developing, writing, implementing, evaluating and revising overall treatment goals and plans in collaboration with the client and the larger team.
Act as liaison and consult with community agencies and families to maintain coordination in the treatment process, including attending medical appointments with clients when needed.
Participate in on-call rotation for crisis intervention/support 7 days/wk, 24hrs/day.
Document client progress to maintain a permanent record of client activity according to established methods and procedures.

Physical Demands and Work Environment:
Physical Demands: While performing the duties of this job, the employee is occasionally required to walk, sit, use hands to type, handle or feel objects, reach with hands and arms, bend, walk up and down stairs, talk, hear, and see. Employee must be able to occasionally lift/move up to 50 pounds.
Work Environment: Most work is performed indoors.
Requirements
Qualifications Required:
Registered Nurse with New York License
Minimum one year of psychiatric nursing experience
Minimum one year of experience in a clinical psychiatric environment
Excellent organizational skills
Good communication and interpersonal skills
Ability to prioritize tasks
Ability to multi-task
Dependability/Responsiveness
Must have skills and competencies to establish supportive trusting relationships with persons with severe and persistent mental illnesses as well as family and other members of the clients’ network
