Location: Visits across Orange County, Los Angeles and Inland Empire

Schedule: Part Time

Compensation: $45- $55/hour commensurate with experience

Position Summary:
We are seeking a compassionate and skilled Registered Nurse to join our Palliative Care Program. This role focuses on providing holistic, patient-centered care in various settings, including patients' homes, assisted living facilities, and skilled nursing facilities. You will play a vital role in delivering high-quality palliative care, supporting patients and their families during challenging times.

Key Responsibilities:

• Creatively manage resources and workflows to triage patient needs and address clinical demands.


• Coordinate with the interdisciplinary palliative care team and referring providers to implement effective care plans.


• Facilitate connections with complementary therapies (e.g., PT/OT, massage, music therapy).


• Assist with non-pharmacological interventions to alleviate symptoms and enhance psychosocial coping (e.g., meditation, Reiki, journaling).


• Support patients and families in legacy work to aid end-of-life coping.


• Ensure continuity of care and consistent team presence across rotations.


• Engage in advanced care planning discussions and assist with completing advance directives.


• Deliver nursing care in patient residences and document assessments within the RN scope of practice.


• Act as a resource for patient care units, outpatient clinics, and staff regarding palliative care.


• Provide education to staff, patients, and families on symptom management, coping strategies, and end-of-life care.

Qualifications:

• Active and unencumbered RN license in California.


• Minimum of 2 years of nursing experience.


• Palliative Care experience a PLUS


• Associate’s or Bachelor’s degree in Nursing.


• Community-based nursing experience, including home visits, preferred.
