Registered Nurse - Primary Care RN

We are seeking an enthusiastic and dedicated Registered Nurse (RN) for our Primary Care team. In this role, you will provide exceptional nursing care to patients in a primary care setting, focusing on health education, patient advocacy, and the management of chronic and acute illnesses. You will work closely with a multidisciplinary team to deliver patient-centered care to improve overall health outcomes.

Key Responsibilities:

• Conduct thorough patient assessments and triage patients efficiently.
• Develop and implement individualized care plans in collaboration with the healthcare team.
• Provide health education to patients and their families regarding chronic disease management, medication adherence, and preventive care.
• Perform routine nursing procedures such as administering medications, injections, and immunizations, as well as assisting with minor procedures.
• Monitor and document patient responses and progress in electronic health records.
• Coordinate follow-up appointments, referrals, and community resources for patients as needed.
• Participate in quality improvement initiatives to enhance patient care standards.
• Work collaboratively to ensure continuity of care and promote a healthy lifestyle among patients.

• Active and unrestricted RN license in the applicable state.
• Associate's Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN).
• 3+ years of experience in a primary care or outpatient clinic setting preferred.
• Current CPR/BLS certification (AHA preferred).
• Strong communication, organizational, and patient advocacy skills.
• Ability to work in a team-oriented environment and adapt to a dynamic healthcare setting.
• Proficiency in using electronic health record systems and technology for patient management.
• Must pass a government background check.
• Able to provide 3 professional references.

• Schedule: Monday–Friday, 7:00 AM – 3:30 PM
• Pay: $32.00 – $35.00 per hour (all-inclusive)
