Pay Rate: $48.00 – $49.00 per hour
Work Hours: Monday – Friday | 7:30 AM – 5:30 PM
We are seeking experienced Med-Surg Registered Nurses (RNs) to join the Referral Coordination Team (RCT) at the VISN 19 Rocky Mountain Network in Glendale, CO. This position plays a critical role in managing care coordination between veterans, VA providers, and community healthcare partners.
As an RCT RN, you will be responsible for reviewing consults, facilitating care coordination, and serving as a clinical resource to both patients and multidisciplinary teams. This opportunity is ideal for nurses with strong clinical judgment, communication skills, and a commitment to improving access to quality care for veterans.
Conduct clinical reviews of consults and care requests for completeness and medical appropriateness based on utilization guidelines.
Determine the most appropriate care setting—within VA facilities or community providers—based on clinical and eligibility criteria.
Manage incoming medical records, ensure proper documentation, and facilitate provider communication.
Serve as the central coordinator of care, collaborating with veterans, VA providers, nurses, administrative staff, and community healthcare organizations.
Triage specialty consults for clinical appropriateness following National Referral Coordination Initiative (RCI) standards.
Educate and assist veterans in making informed decisions about their care options, location, and scheduling.
Act as a clinical liaison and trusted resource to multidisciplinary staff including MDs, LIPs, Social Workers (SW), and Health Administration Staff (HAS) across VISN 19 and beyond.
Partner with Clinical Authorization Coordinators (CACs) to improve and revise consult templates.
Develop strong working relationships with VA specialty providers to support effective implementation of RCI tools and practices.
Serve in a Delegation of Authority role within the consult process, advocating for the veteran’s best clinical pathway.
Active Registered Nurse (RN) license in Colorado or compact license eligibility.
Minimum of 2–3 years of Med-Surg experience in an inpatient or outpatient setting.
Strong understanding of care coordination and utilization review principles.
Excellent written and verbal communication skills.
Proficient in using Electronic Health Records (EHR) systems and documenting clinical notes.
Experience working with veterans or within the VA healthcare system.
Familiarity with the Referral Coordination Initiative (RCI) and care management protocols.
Background in community care, triage nursing, or interdisciplinary collaboration.
Worksite: VISN 19 – 4100 E. Mississippi Ave., 11th Floor, Glendale, CO 80246
Hours/Week: 50 hours (10 hours/day, 5 days/week)
Shift: Monday to Friday, 7:30 AM – 5:30 PM (Excludes Federal Holidays)
Duration: 52 weeks (May 1, 2025 – April 30, 2026)
Travel & Expenses: Not eligible
